TKR Need better ROM, please advise.

Hi friends,
Here's my 3-month report. I had my total right knee replacement on 06/04/12, developed an infection in my incision and took Keflex for two weeks. PT said I had to wait until infection was gone before starting there. My OS claims I had an ROM of 90 when I left the hospital but now that I was idle for two weeks my ROM was about 30. My OS put me on some strong meds and had a PT at the hospital bend my knee very hard and I almost passed out from the pain. I was screaming from the pain but so much under meds that I was unable to defend myself or resist this gorilla. They sent me home with a CPM machine then I started my PT. Going to PT was a nightmare since my knee wasnt bending good. I would leave that place in pieces after they worked me so hard. My OS wasn't happy with my progress and sent a fax to my PT asking her to be more aggressive so my ROM improves. My knee was so swollen that I thought my incision would open up. After 2 months I only had an ROM of 69, and this was with a lot of suffering. They say a man is not supposed to cry?..I cried after each PT session. I would go home and sit in a corner licking my wounds like a puppy.

All my OS cared about was ROM. The next visit I was so proud to tell him I was at 69 but his answer was "is that all you can do?". He then told me I'd better be at 90 for the next visit. I panicked. For the first time I went to google looking for knee replacement help. I'm 59 yrs old and this is the first time I get hospitalized. I had never looked for help before but now I felt the need to get ROM and get it fast. I was given exercises to do at home and i was doing them constantly plus my PT was agressive. I was hurting all the time. The only good thing is that i was given vicodin 500 mg ( but it didnt come in six-packs) lol. Once I came to Bonesmart all of a sudden I find out I'm going in the wrong direction. Imagine 2 months doing the wrong things. Enter Bonesmart.

I was welcomed and told help is on its way. I then started getting all kinds of advise. I read each message slowly and it all made sense. I learned I was shocking the knee to the point that there was so much trauma it was causing it to swell more therefore I had to do the opposite. Since then I started elevating and icing a lot more, followed some of the examples I was given by other members:
Bought a used exercise bike, couldn't pedal around but I learned to pedal backwards and now I can, did a little walking more often, slow, gentle stretches, holding for 30 seconds 3 times a day allowing no more than a pain level of 3, on a 1-10 scale. Elevate, ice every 2 hours or close to it, massage my scar with E vitamin oil, move my knee cap up, down, left, right. I listen to my knee, knee is the boss and it knows when enough is enough.

All this I learned from my new family at Bonesmart the last month. My ROM is now at 95!!!! When the knee is so swollen it just wont give. Doing more aggressive PT upsets the knee gods! Ice, elevate as often as possible. I went from 69 to 95 in a month. Last Thursday I had a visit with my OS and I proudly said I was at 95 and he said I should be at 110. I told him I was quite happy with my ROM. (what is it with this man?). My PT has seen my improvement since joining Bonesmart and now I have "trained" her to be gentle.

This Monday I go back to work (bills wont wait) I have a desk job so it will be interesting. My OS provided a note I must get up for 15 minutes every hour and walk around. I will let you know how it goes. I will be doing some exercises I learned from member David (Kneesrus) which are great for basic or advanced workouts. The library also has plenty of info.

I want to thank everyone here for all the advise I have received. I wouldn't have improved so well without your ideas. I'm not afraid to go to my PT anymore and I know next month when I go to my OS my ROM will be even better. I may not see him again after that.

Be gentle to your knees!

Regards,
Al

Al, here's all the high fives, cheers, kudos, and congratulations you should have gotten from your OS (shame on him). We here at BoneSmart applaud your stepping up to the plate and taking charge of your recovery; it has paid off big time!

Please keep us updated on further progress, we care.

Al, I was so happy to read your note---thank you for writing it. It is rewarding to help people---especially when we see people going down the wrong road!!! Another month of torture would not have done it for you. and, I am so glad that you finally spoke up to that doc who can;t seem to make a positive comment to you.

You are doing so well, I am sure your rom will improve for the next six months, so don't worry about that right now.

If you can ice at work, it is worth the effort to bring a lunch bag or something with ice in it. And, many people put a box or stool under their desk so that the leg is elevated a little and not down too much. I wore complression stockings--not ted hose, but like them to work for the first tkr and it helped to control the swelling. They do make them for men too. I wore thight highs which ought to get you a new reputation at work!!!

I used to go and lay down with my knee on a chair at lunch too--anything to get some relief for the swelling. There will be lots of folks to give you work tips. congratulations on taking your recovery into your own hands and finding success. Kelly

Thanks Kelly,
I'm very excited about my improvement. It feels good when you can finally put on your socks, drive your car and put your feet under the table during dinner. That, to me, is HUGE. Im more positive since Ive learned what to do now, thanks to everyone here. I do have my compression stocking still so I will use it when I go back to work. I also intend to bring a sealable food storage bag and fill it with ice from the cafeteria to cool my knee. I have a filing cabinet about 3-feet high, with a cushion on the top and I will use it to elevate my knee. Any other ideas are welcome as work will be my next challenge.

You confounded your surgeon then, eh? Good for yo. And a testimony for many others whose surgeons are so bullying to take note of.

Wow...great report Al! I am so proud of you....you are doing great! Good luck going back to work...take it easy and ice and elevate and you should be just fine. Remember it will only get better and around here in TKR land slow is the way to go!:)

:thumb: Walking around for a few minutes a couple of times per hour is good. I know I found it helpful.

Well folks, today was my first day back to work since May 29th. It was the longest 8 hrs of my life! I elevated my knee as much as I could, I had ice on my knee twice and was given 15 minutes every hour to walk a bit. I also walked around the building during my one-hour lunch . The temperature was only 106 today, last week it was an average of 117 degrees. i work in tech support in a call center but this week they have me catching up on training and reading emails. That made things easier. It took me a few minutes to access some programs because I couldn't quite remember the codes. Of all the things I've lost I miss my mind the most!! The codes came back after I went to a folder to find my cheat sheet.

My legs are tired and my knee cap is sore. I have no desire to go near the exercise bike since I feel I don't have any energy left. I'm ready to take a shower, massage my scar with vitamin E oil, take my Vicodin and go to sleep with my leg on two pillows. Has anyone else felt that way? It was nice having all day to exercise and go to PT. Now I have to manage my time well from the minute I come home to the time I go to bed in order to continue gaining range of motion and being gentle to my knee. I need your suggestions. Tonight I'm only icing and elevating because it's my first day back. I will do better tomorrow.
Regards,
Al

Just keep up the icing elevating and getting up and moving around on a regular basis. You might just stand at you desk for a couple of minutes every 15 minutes or so. That keeps the stiffness and swelling down also! Great report by the way! :happydance:

Remember, Al, that all the activity involved in getting up and getting to work, being at work, getting back home---is all exercise--it all helps with ROM. And, of course, you are exhausted by the time you get home. You do not need to do additional exercise until you build up more stamina---you're getting a plenty right now just living your life. You need to rest, elevate, and ice. It's going to take a while----TIME & PATIENCE---TIME & PATIENCE---TIME & PATIENCE. You'll make it, but it's going to take TIME & PATIENCE.

Al, I'm so happy to read this awesome report!!! You and I had similar beginnings, right, Friend? I'm going back to my OS today (for the last time, as he's retiring!!), so we'll see. He's never been supportive in any kind of way, only the people on this site have.

Again, I'm so very happy you've found the help you needed!!

Hugs, Liz

You may need to come home and flop and ice and elevate for the first week or so. Also, are you really walking around for a whole 15 minutes every hour? You probably only need to walk around for a few minutes to loosen up.

Wow! I used to live in Phx, and I 'get it' when you say it was "only 106"!!!
Remember that going back to work is a huge shock to your knee, and your stamina. I would EXPECT you to go home, ice, elevate, medicate and hit the sack!
I agree that you only need to stretch and walk around for a few minutes, but better your boss not know that!
Don't over-do the lunchtime walks, EASY is best!
Good luck, Sandy

Sister Liz!
We've been in similar situation but we both found the right place to find the right help we both needed. I was reading my posts last week and I cant believe the difference in my attitude since I joined Bonesmart. I was hurting, desperate for inmediate ROM to appease my OS demands at the expense of my poor knee. Im doing great now. I also feel my knee and I are a team. ROM will come as Im being gentle to my knee. Let me know how the last visit with your OS went.

Kneeper, yes Im taking about 10 min to walk to the cafeteria and back. Do you think maybe Im overdoing it since this is my second day back? Maybe just standing and stretching should be ok? I had another long day but a little less painful. My knee feels tired but my knee cap isnt sore this time. I may be ok by Friday. I like the idea of medicating, cing and elevating with some stretching for the week.

Sandy, todayit rained cats & dogs since the temp was only 106. We had a monsoon with high winds and lightning followed by heavy rain for about three hours. I didnt do the lunch time walk. I sat in the cafeteria and elevated my knee on another chair and iced the knee for 20 min. I wi do the same the rest of the week.
My swellinis going down some more. This is great. Amazing what great results one can get when given the right advise. Wouldnt it be nice to be able to meet everyone in the forum and personaly thank them? I owe my progress to all the advise you all gave and continue to give me.
Take care of your knees,
Al

I'm glad it's working so well for you, Al. Glad we could help, too! :)

I agree that you only need to stretch and walk around for a few minutes, but better your boss not know that!

True--getting the ok for 15 minutes gives you flexibility.

Hi Al! I also had my surgery, both knees, on June 4. I was fortunate to find Bonesmart a few weeks before my surgery. I'm so sorry about your two months of unnecessary agony, but am thrilled for you that you have turned your recovery around! You are doing just great now! I'm sure work will be a challenge, but it seems you have some flexibility there. Sitting is just starting to be more comfortable for me, where I can sit at a restaurant through waiting, ordering, waiting, eating, waiting, paying the bill and finally getting up to leave. It all comes with time!

You sound basically like a happy camper.

And you're wise to come home, ice, take the nice drug, and prop up your leg. Thank goodness someone knows how to give your knee TLC. It needs it!!

Congratulations on your success.

Bottomsholla, I somehow missed your comment and now that I found it it makes a lot of sense the way I'm feeling now that I just went back to work Monday. You said it best:

Remember, Al, that all the activity involved in getting up and getting to work, being at work, getting back home---is all exercise--it all helps with ROM. And, of course, you are exhausted by the time you get home. You do not need to do additional exercise until you build up more stamina---you're getting a plenty right now just living your life. Truer words were never said!!! No wonder I've been feeling exhausted! I'm doing more exercise than ever. Never thought about it that way.

Janet, we both had surgery the same day but you had both knees. Wow. I'm glad you're starting to get more comfortable. I have been using a toilet seat extender to make it higher. I went to work Monday and realized I can now sit without it. I probably didn't need it last week but now that I saw myself without it at work i was forced to try it. We will be ok. Time & patience is the key.

Thanks Maryo. You're right, ice, meds, elevate, have some Maine lobster bisque and relax!

Liz, I will check your post. Can't wait to read your report on your last visit to your OS.

Be gentle to your knees!

Al
