TKR Blondie 58's recovery thread

Sue, if you are experiencing swelling with your knee, you may need to be attempting a bit too much too soon. Can you please tell me what activity, exercises (what kind, reps, how many times a day), household chores, errands, etc. you are doing in a typical day? Are you trying to be up more than you should? Skigirl is right on with the "two hours rest for every hour up" recommendation.

Hi Jamie, thanks for your post....

OK, well i'm almost 5 wks post op & have been looking after myself since week 1 (no choice unfortunately), although I do have close friends who help me with doing my washing & the odd bit of shopping & visiting, I do a regular shop online every 2wks & the delivery boy very kindly brings it right into my flat, although I have to put it away myself which always exhausts me, not to mention kills my back!! I cook for myself, nothing too fancy, porridge in the microwave for brekkie, usually a wholemeal sandwich for lunch & I do usually make myself something for dinner, i.e. an omelette, stir fry etc; although i've learnt to sit down whilst chopping veg/preparing stuff as I can't seem t stand for long periods, it kills my back as well as my knee? I do my own washing up, run the hoover round quickly initially sat down as my flat is very small & do the rest standing up later, I invested in a light, cordless vacuum before my op as it was killing my back before surgery (my friends will do it for me if I ask, but i'm a bit proud!!). I shower, albeit every other day, daily is still too much for me, wash my hair etc & started to venture out to my local shop last week (at 4 wks) it's literally right behind my flat 15 metres away, i've been able to manage the flight of 12 stairs leading up to my flat since wk 2 & without crutches for the last week, although if I do go to the shop obviously I do stil take my crutches with me, I still can't walk outside without them, although I can potter around my flat without, it's so tiny. As for exercises, I don't stick to a regular regime, my thinking is that my knee does enough tbh, the bends are made really difficult as I said due to swelling, but I do the foot wiggling & gentle stretches & my pT was pleased with me enough last week anyway & said the swelling was normal at this stage? Having said all this I do spend a LOT of the day just resting/elevating etc on the sofa or in bed.

I hope this helps.

Sue.

Hi @Nana moon 27!!

Thanks for your message, what you've said makes so much sense & I know you're right, the problem is I was on the strong narcotic meds 3/4 yrs before I had my surgery to help deal with the pain predominately in my ankle & subsequently my arthritic knees, so that when I did have my LKTR done, they weren't enough to cope with the pain & the Hospital had to increase the dosage on some of my meds just to take the edge off the surgical pain & they stil only just do now. I'm in a catch 22 situatioin, but i've decided to give into my body & just lt it sleep whenever it wants to, I don't seem to have much choice!!

I would just add that since I 'dipped my toes' so to speak into these Forums, they have been invaluble & I look forward every day to coming online at some point now-thank you everyone!! :sneaker:

Just do try to keep the things you're doing minimal...especially those household chores like vacuuming. The world won't end if your flat isn't kept to its normal state of cleanliness during these first weeks of recovery. The thing I see missing is icing. Ice is wonderful to help with pain and swelling. I sugges you ice your knee whenever you are resting and elevating. You can even ice when you sleep. I found that to be very comfortable and a big help for sleep.

Hi @Jamie,

I know what you're saying & you are quite right. In hopsital they tried icing several times but the pain was too bad & I couldn't bear it. Now, practically, the problem I have is I only have room for a camper-type fridge in my tiny apt, & consequently the small ice compartment is no good for making ice in the quantities I would need for my knee. However, strangely enough, the 2 gel ice packs I ordered from Amazon arrived just today (less that £5. for 2 & amazing quaity!!), & as I type I am icing my knee, boy it feels good too!! They just fit in my freezer box nicely, & I can tolerate it now-I asked my PT last week when I ordered them & she confirmed it was never too late to start icing, so hopefully the swelling should now start to dissipate, somewhat at least?

Thanks for your concern though Jamie, it's greatly appreciated & your Site has been a godsend to me in my darkest hrs, yes I have them even tho' I may seem upbeat most of the time!!

Kind regards
Sue.

Good for you. The gel packs are what I preferred. Use the heck out of them. You can also ice the back of your knee and that sometimes is just as good or better than laying the pack on top. Or....you could use two gel packs so the ice is all around. Whatever works best for you.

Hi @Heyblondie58

I was also on strong narcotics before my surgery too. I'm still on them but a much lower dose. I was put on them because nothing else would work on me. I dislike them. I weaned off them twice and got told off really bad when I showed up at my GP office in extreme pain. He and my OS refused to let me off them.

Everyone's pain is different. Our individual recoveries are exactly that. Jamie's right, housework can and should wait until you're more mobile.

Take care
Love Ann xx

@Pookie2015,
Thank you for your lovely comments, they're very much appreciated right now!! :thankyou:

As regards meds, I take 50mg MST (Morphine Slow Release) bi-daily (every 12 hrs, 6am/pm), 200mg Gabapentin x 3 daily, & Oramorph (liquid Morphine) as & when for 'breakthrough pain'.

Sue.

After my BTKR, I used these---ColPacs---12"X18"---they worked wonders---a friend of mine just had arthroscopic surgery, and her leg was swollen, and these helped her out a great deal.

Hi @referee54,

Many thanks for taking the time to post this tip-however, these will not fit in my small freezer box (see above), so i'll have to stick with the smaller Kool Paks that I bought off Amazon. They are, however, extremely effective & I can't believe in just one day how much they are helping my knee to feel better, & the cool sensation is just awesome & very addictive!!

This Site is so friendly, I feel at home already, once again a very big :thankyou:

Sue.

Hi @Nana moon 27

I know what you mean about trying to wean yourself of the narcs, i've tried a couple of times & just end up in pain, in tears, desperate, & my GP tells me the same as yours probably did-if you need them take them & why suffer etc etc;
I've decided not to stress about my meds now for the time being, god knows us newly operated 'Skneezers' (my new nickname lol) have enough to worry about without beating ourselves up about what we're taking for the pain!!
I'm unable to tolerate any of the NSAIDS, & had a very bad reaction to Tramadol the 1st time I took it, so i'm very limited as to what I can take for pain relief, Codeine isn't strong enough for me now, so, unfortunately, Morphine is really my only option.

I'll take the slap on the wrist from you & Jamie re the hoovering, I knew I shouldn't really be doing that...:iagree:

Thanks for caring Ann, what a lovely bunch of people!

@Heyblondie58 , I just had my 2 week post-surgical check today with my OS. My knee is still pretty puffy and swollen in spite of lots of icing and elevating. I asked the OS, the resident, and the nurse all separately about the swelling and they all gave me the same answer, "It will likely be swollen for 3-6 months minimum." Eventually it will get better. So, unless it's swelling worse after activity, I don't think I'd worry too much about it at this time. Mine is just basically puffy all the time.

@Pookie2015 , my surgery was May 1 so I'm a little behind you. I take one percocet and one regular strength Tylenol every 6 hours around the clock. That means I only use 4 percocet a day and can take an extra tylenol or two without going over my limit if need be. I just got a new refill for 80 percocet and doubt I'll need anymore.

Sweet dreams all,
Roxie

@Heyblondie58

I can't say anything really about hoovering when I was at your stage of recovery before....I did the same. I'm obsessive with the vacuum xx I paid for it though!!!

When I have my second knee done in just over a weeks time, I know what NOT to do x

Anyway, I'm glad you got your cool packs off Amazon. The icing will help immensely xxx don't forget elevation at the same time.

You're doing just great Sue x

I am a "clean freak", but the pain meds that I was taking after my BTKR had a strong effect on me and I did not run the vacuum cleaner for a while. I figured that the dust bunnies would not be going anywhere, and there was a time for me to catch up to them down the road. Finally, my kids came home from the university and helped catch up on the cleaning that my darling bride could not get done.

Hi guys,

Hope everyone's making good progress with their recovery, sorry i've not been online for a few weeks, have had problems with infection in my scar around 3 wks ago & it's taken 2 lots of antibiotics to clear it up, the last time I had an infection was after my hysterectomy 16 yrs ago, I have a very strong immune system & normally heal without any problems, but there were 2/3 staples at the bottom of the scar when I had them taken out that were slightly overlapping, red & gaping (Community Nurse noticed this & told me to keep an eye on it) & guess in the end I was just unlucky:bawl:& have had to rest up quite a bit more than I would normally :elevate:.

However, despite this, I am now 9 wks post-op & my OS was extremely pleased with me at my 6 wk check up, he said I was 'showing off' walking in without crutches lol & my ROM then was 90'/0'. Fortunately, he's lovely & NOT on of these OS' that insists on a 120' flexion at 6 wks & apparently, athough the flexion/knee bend IS important, the extension of the leg is the thing they look for & (miraculously?!!) mine has been at 0 since wk 4!! I've still managed to keep up my excercises & short walks etc (despite my setback) & at the Knee Club Physio Clinic that I attend whenever I can on a Monday, it's Voluntary but I prefer it to the 1-2-1 Physio sessions, as there are other people in the same boat, &, like this Forum, all at different stages of their recovery, so there's the social side too, as I can't exactly go out clubbing just yet!! :dancy::spin: Anyway, I went this afternoon & was pleasantly surprised (not to mention very chuffed with myself!!) that my ROM has now upped to 105'/0'. :wowspring:!! That may not sound amazing to you some of you at 9 wks, but all the medical guys involved with my care/physio etc; think so & i'm quite happy with my progress thus far, so that's all that matters. :happydance:

I guess the point I am trying to make to you lovely people on here is that less is actually more with this recovery journey. If you push yourself too hard to ride bikes/speed walk for miles every day, you're actually doing more damage than good, & almost certainly letting yourself in for many depressing disappointments with unneccessary swelling, pain, not to mention the stress of achieving unrealistic goals by pushing yourself too hard too soon & then being both disapointed & actually worse off for it?

I'm far from healed myself yet tho', still spend half my time resting & icing (alternating with heat), I still have lots of internal bruising/swelling & the 'heavy elastic band' feeling & stiffness whenever i've sat down for just half an hour or first thing upon waking, but am down to one crutch now. I walk around my flat without any support, but due to the complication of my right ankle fusion which will never heal (see my original post) I will most probably always have to use one to walk outside.

My main problem now is my right knee-I don't want it done, but I desperately NEED a RTKR & sooner rather than later. :groan: When I tried to do some 'wall slides' at the knee Club today, it was almost impossible for me to get up afterwards, & even my Physio advised me to stick to the exercises that involve just the operated knee, as the grinding, cracking & grating noises from the right one were even making her squeamish, so you can imagine how I feel, the pain in that one is getting unbearable & the swelling has increased, it's also affecting my knackered right ankle, swelling pain etc; etc; etc; All of this is naturally due to the right leg having to take most of the weight off the operated knee since the surgery, i'm sure many of you will identify with this? Another very uncomfortable symptom i've been suffering from since day one is a burning, tearing like sensation just beneath the incision, it almost feels like I have another scar underneath the external one that is trying to burst open, it's agony at times & throbs a lot too, OS said it was perfectly normal & that it was all the damaged tissue & nerve endings trying to heal & my GP increased my Gabapentin (which I have for the permanent nerve damage in my right foot ) to 600mg thrice daily with Oramorph to take as & when for 'breakthrough pain' he told me to phone him anytime I needed to talk & to hold off as long as I can for the right one to be done, but not to let it get too bad & would be guided by me & I think i'm going to be calling his Secretary to be put on his list in the very near future, I just want to get it all over with now, so that I can (eventually) return to some sort of normality, whatever that may be!! :rotfl:

Sorry for rambling here, my update seems to have turned into War & Peace lol!! :type:

Looking forward to hearing from some of you with your updates?

Sue

Hi @Heyblondie58

What a lovely, informative and useful post you just made. I'm to hear about the problem you had re your scar but you're sounding so lovely and positive.

Your ROM is fabulous. I'm so pleased to read how you've achieved this.

I so related to your post about the group physio. I had exactly the same problem as you. There were so many exercises I couldn't do because I needed a LTKR. I've had it done and although my recovery is slow (I was warned about this by the OS) it had to be done.

9 weeks no crutches is fabulous and nice that you're down to one around the flat as is necessary. Well done. Keep that lovely recovery coming and please keep us updated. It's a pleasure to read it x

Hi Ann!!

Thanks for your lovely reply to my post!! However, i've just noticed that you had your 2nd TKR done only 2 wks ago, wow that was very brave so soon after the revision on your right one....how are you doing, are you in pain, was it worse than the first op? I've been told also that because of my ankle complication that my RTKR is going to almost certainly take a lot longer to recover from than this one, & i'm prepared for that, that was the reasoning behind operating on the left one first, but like I said, i'm NOT looking forward to it, but I need it done desperately as i'm so limited with this right leg now & the pain is just getting rapidly worse!! :bawl:

Hi @Heyblondie58

I had no choice but for this one to be done. It felt like it was broken. It's a relief not to have that and the dreadful bone on bone pain. Yes there is the usual surgical pain and although I'm having an off day today, Ive no regrets about having it done. I need to get my life back now and this is my way forward. The revision was fabulous in the end. It took a while but it got there. The 1st TKR has turned out to be a lot stronger than I ever thought it would. It's like a normal knee now. Yes I get the odd ache and stiffness if I sit too long but it's holding up well.

I think that psychologically, the second time around, I'm in a better place because I knew what to expect. Having this site for support is a real useful help. You don't feel so alone when you know that there's always someone around when you can't sleep or your anxious and have a query about some aspect of recovery.

I'm sorry to hear about your ankle complication. Only you can make the decision about this TKR. It's an important one because of the added complication. I would suggest that you recover fully or as much as you can from this one and then perhaps consider it if that's what you decide to do. I think that the advice given to you by your OS is sensible and right.

Please take care Sue
