THR I am titanium!

Just wondering if you were diagnosed with spondylolisthesis at some point? My husband has this issue and definitely has referred hip pain from it.

Hope you are feeling ok today. Chronic pain is such a grind!!

Hi, @calikat ... I wonder about you and how you are doing. I'm not on bonesmart much anymore, but you are in my thoughts so often and I hope things have improved. Thank you for all the support you gave me. Please take care.

Did I just see you mention in Tiona's thread that you are having a problem with your cup? Are you facing a revision surgery?

Hi @C Mac (Cardie). Yes, I have seen two additional hip specialists and both agree that the cup is not in the optimal position and I have an overhang. One surgeon recommended trying to live with it and offered me more pain medication. The other, @Hibbe's surgeon, thought I should fix it. He called me at home to go over the results of my CT scan. He would revise the cup and change out the components of the socket but leave the femoral component in tact. He said the surgery would take about 45 minutes. He thinks that without the revision I will continue to have pain because the cup is misaligned and rubbing on a tendon. I scheduled another appointment with him this month because I have many questions and want to gather more information. It is a big decision. In the mean time my other hip is really starting to be symptomatic although the artificial hip still hurts more than my real one.

So sorry to read of your pain @calikat - I like the sound of this surgeon who rang you, caring!

It is a difficult decision, but sounds as though more medication is not a preferred choice.. Too much living to do honey!
 
I know I was impressed too @Poppet. He called me on a Thursday night around 7 PM, but I missed his call. He called again the next night around 5:30 and spoke with me for about 30 minutes. He did not think I should just "live with it" either. But I will admit that I am afraid of making it worse. With daily medication it is manageable. I take a celebrex in the morning and an ultracet at night. If I don't take the medication I am pretty miserable....lately in both hips...LOL! I try and experiment with taking less, but the hip is having none of it. :/

I am curious to talk with people who have had hip revisions. I have to have the cup of my THR cut out and revised because it is misaligned. Is there anyone out there?

ETA I have been looking in the revision section, but there do not seem to be many success stories. :(
 
@calikat Sorry to hear that you are having to go through revision surgery .....I can only imagine that it is some sort of relief to get something done after all you've been through but coupled with anxiety & worry about the outcome.
Strangely, I met a woman on the beach yesterday whilst walking my daughters dog.....she was walking a very lively dog and we got talking as she noticed I had my cane with me (which I still use for longer walks on uneven terrain). She asked why I used a cane smiling knowledgeably when I mentioned THR.
She went on to tell me that she had a THR 10 years ago and that all was OK for the first 5 months and then she noticed her hip sticking & clicking......with an increase in pain. She was told that this would disappear but it got steadily worse over the next 6 months.... The upshot of this was her surgeon recommended a revision (a year & 2 months after her THR). She said her recovery was much quicker than the original op and she has had no problems since.....she had no walking aid with her either! She was 65 when she had the revision so must be in her mid 70's now and was very mobile.

@calikat , lots of members have had revisions. Please don't take the bad experiences as the norm. The nature of a forum like this is that the members with problems stick around longer. Most people are off living their lives post revision and just don't have the time to post. One member who might give you some input is @Z15 - that is unless he is off jogging or hiking somewhere fantastic!

Most recently @Greg had a revision and he seems very happy with the result.
 
Thank you for the replies. I have been trying to see if I could "live with it" as one doctor suggested, but I have pain all the time which recently has started to worsen. I have decided to move forward.

So glad that you have opted for the revision. It would be awful to try and live with pain like that and it would only get worse with time. Just be sure you have an experienced revision specialist and you should do just fine.
 
@calikat Hello there! I am sorry to see that you need a revision. I am on day 13 and its too early to scream "success" but I think things are going pretty well. Its odd because some things are better than last time and others a little worse but I think overall its better. I dont have nearly as much swelling but I have a little more pain but I think thats to be expected. Also, I can't completely weight bear yet due to restrictions but I did start to test it a little and I can tell once I rebuild the muscle I will be good to go. my biggest problems in the first week post hospital were things like poor communication with the nurse at my Dr office or the time I slipped on my steps and thought I messed up but false alarm. But thats me kind of, if I am not running laps on day 5 I am uptight :) ha ha

Ftr, my revision was due to metal poisoning from a metal on metal implant by Biomet. We were able to use the stem but had to replace the head and cup and liner. I now have metal on Poly.

I will be glad to help anyway I can. Make sure this surgeon is a "revision specialist" as you probbaly know.

So I saw the famed Dr. Joel Matta today. I spent quite a bit of time in his office while he and his fellow measured, drew, discussed, measured some more took new X-rays etc. He said he hates to recommend it, but he agrees I need a revision. He said my original surgeon is well respected and the result is unfortunate. I said I know things don't always go the way they are supposed to, but said I don't have faith in the original doctor fixing it since he does not even think or won't admit that anything is wrong. The three other Os's I have seen all agree there is a problem. My acetabulum component is highly anteverted and when I move my leg there is impingement (Dr Matta's assessment) . There is also subluxation (?) when I turn out my hip which they noticed on the X-ray and CT scan.

Dr. Matta said he said he does not really like to do revisions and does not do that many...about 2 a month...although he just did two last week. When I asked him if he could refer me to anyone else he said he did not think any one else could do as good a job as he could. lol He felt I should stay with the anterior approach since with a revision I have a higher chance of dislocation. But it seems my hip is already partially coming out of socket (I believe that is what subluxation is?) despite having had an anterior replacement. They moved my leg around and felt the odd movement I feel in my hip. It kind of wiggles and jerks with certain movements. So they basically said the same thing that the other OS who originally recommended revision said...although he uses the PATH method. Dr. Matta ended are appointment with "Well you definitely have a problem." I had to laugh. He was very nice and thorough. He unfortunately does not take my insurance...I paid cash for the consult. Oh and my leg is about a centimeter shorter than the left. I already knew it was shorter as I can feel it, but just not how much shorter.

I now feel more confused as to which doctor to choose, and which method, but less confused about whether I need a revision although the thought of having one frightens me. Dr. Matta also felt that the polyethylene component will wear faster and cause inflammation and damage to the surrounding tissue which he thought might be some of the pain I am feeling now. I often have a burning sensation inside my hip and it radiates down my leg. I told him I wish I could see what is going on inside there.

At any rate, I am feeling sad, scared, and disappointed. One thing all three OS's have said is that a revision is never as good as a primary...although my primary is not good so maybe that is a moot point...and that there are increased risks etc.. Dr. Matta mentioned a few scarier complications than the other OS...bone breakage etc. when they cut out the cup.
