TKR 5 Weeks Post TKR

To add to the previous comments, here’s an article from the Bonesmart Library, about that tight band around the knee.

I’m glad your doctor discussed your hip issue. I, also, have hip issues these days and I thought I was headed for replacement, but a recent X-ray showed very little arthritis in my hip, however my chiropractor said, my spine was a different story! And, like you, I also have some scoliosis.

Yesterday my internist touched my hip spot where I experience the pain and said it’s a bursitis type of pain . Definitely inflamed and recommended stretching and exercises to strengthen the glute!! I was in for every 6 month checkup and mentioned the hip .

Even though the diagnosis from your internist is slightly different than what you heard from your surgeon, the idea that you may need to strengthen your gluteus muscles is the same. So, hopefully your therapist can give you some exercises that will help.

Have a look at my pilates teacher's free YouTube video to strengthen the glutes.
It is a short side lying/ knee friendly sequence with no squats. It is one of my go to regimes which I used after my PKR.
Google Katja pilates glutes and it comes up.

I have suffered with the inflamed hip bursa for over 5 years. I am going to try the site that EalingGran suggests. I pray it works for both of us.

Thank you for your advice I will!! But I will wait till pain free to start!
Wednesday after no PT for 2 weeks and much reduced hip and knee pain, today I’m in pain again after the PT session. I told him the PT what OS said I had, but at this point No more squats, clamshell, riding bike etc as all seem to exacerbate the pain. My OS told me I didn’t want to test the tendon so for now I’m taking it easy.
I’m done with PT!!

I am sorry for your pain, ExRunner. Hopefully you're able to gain some relief through, EalingGrans, suggestion.
Thankfully you're stepping away from therapy that is causing you pain and exacerbating the issue. Please keep us posted on how you're progressing. Best Wishes for a pleasant weekend.

Today I went to my pain management Ortho who injected my other knee so I’ll be functional in Italy.
My TKR knee is improving as I can walk long distances now and I bend better. Occasional band feeling but even that’s getting better. I’m 17 weeks out!!
Now my hip pain is another story as I found out today at pain ortho office that I have medial gluteus tendonopathy!
If I aggravate it it could tear!
Apparently it’s the tendon attached to the muscle that attaches over the trochanter.
A heating pad works and is recommended over ice and to do specific exercise as long as they don’t exacerbates the injury .
I read having scoliosis plus one leg slightly longer than other can cause this and so can certain PT exercises!! I’ve thought PT created this all along !

I am sorry for the gluteus tendinopathy, ExRunner. Hopefully the suggested treatment brings relief and the condition doesn't get any worse. Thankfully you're making steady progress with your right knee and you'll be golden for Italy. :fingersx: Happy holiday weekend!

Well 3 hrs sitting on a heating pad last night finally made my butt hip feel better!!! So it appears heat is the way to go!!
The help and support I’ve gotten from this forum is like no other . I’m grateful for everyone’s comments !! You are the best!!

Great news! I'm happy you're feeling better, ExRunner.
Thanks for sharing your journey here and for your support of others. In addition, thank you for your kind words in regard to BoneSmart. It is appreciated! :)

Well here I am 24 weeks now post RTKR , in Italy at my family home doing great with my knee but experiencing bilateral butt / lower back / hip pain that comes and goes!
I think it’s related to my TKR leg being slightly longer as I’ve said before but this is now limiting my activity level due to the pain . I’m taking Advil 600mg every 6-8 hrs , using an unknown ingredient 5 day pain patch from the pharmacy here and hoping it stops!
If I move wrong it’s a sharp pain either on the left or right butt area . My pain management ortho said prior to coming and I’m here for 5 months that I had R medial gluteal tendinitis . Well now it’s sometimes on the right and at times on the left .
Anyone else experience this? Advice while I’m here??

For one thing, I want to caution you about the pain meds and patch you’re using. The maximum dose for and NSAID is 1200mg in any 24 hour period. You are exceeding that even if you only take that dose every 8 hours. This puts you at an increased risk for stomach problems or bleeding. Any bleeding can be stealth and you wouldn’t know it. So I really recommend that you not take this much Advil on a regular basis. You say the pain patch has “unknown” ingredients. You mean there wasn’t anything marked on the package about what is in it? I wouldn’t recommend using anything that you don’t know what it contains. For example, if it also contains an NSAID, you are taking even more of this medication than you should.

If you can try Tylenol along with 1200mg of an NSAID, that would be a lot safer and might work.

Try not to do things that cause your hips to hurt (I know, that’s obvious, right?). But that means watch your activity levels that include stairs and walking on uneven surfaces or sandy beaches. See if a heating pad will help. While you don’t want to overdo your activity in a given day, watch how much sitting around you’re doing too. That can be as bad as too much activity. When you do sit, don’t cross your legs. And put a pillow between your legs when you sleep at night.

@ExRunner … if you read my post earlier, I had a typo for the amount of NSAIDs to take. I’ve edited the incorrect “12mg” to read 1200mg in any 24 hour period. Sorry!!!

Thanks for the advice .
I’ll be very careful as medicine here isn’t like the USA unless you go to a University setting Doctor and I certainly do not want to end up at a hospital ER as we are here till end of
Oct.
I know one patch contained
Diclofenac with Brand name of Voltaren changed every 24 hr. Care with using that drug patch or gel is very important as it affects the liver!!
Voltaren is now over the counter in the USA and I caution my friends to be careful in continuous use! I believe it should be black labeled!!
The 5 day Made in Italy patch does not indicate anywhere what the active ingredient/ drug in it is. I scrutinized the package!
I’ll ask my pharmacist when I head into town exactly what’s in this patch but this morning I woke up and turned without that sharp pain although if I swivel my pelvis on the chair now I know it’s still there!
My ortho told me I can take Advil 600mg every 6 hrs up to 2400-3200mg / 24 hr period but definitely not long term.
I do realize that NSAIDs plus the patch can increase stomach issues / bleeding / gastritis plus can also raise liver enzymes as it’s happened to me twice prior to my TKR as I popped Advil like M&M’s to relieve the pain. Once I was off the NSAIDs for a month the liver enzymes went back to normal.
At that time i was using the over the counter Salonpas ( diclofenac) patch daily for 9 plus months in conjunction with the Advil which is a no no!!
Unfortunately my house here is made of stone has stone stairs for which it’s a constant up and down along with outside which is cobblestone .My village dates back to the 800’s .
My knee is great , stiff a bit sometimes but great for which I’m thankful and walking would be perfect if not for the hip butt pain!
I have good supportive sneakers but I'll try to find an insert for my shorter leg side.
The pharmacist yesterday mentioned I lean to the left perhaps that strains the right butt / hip area hence causing the pain.
I googled hip butt lower back pain and I guess it’s more common than I thought after TKR! Today I’ll buy a heating pad and try that too !
Again thanks ….

Yes, it really is. I had this with both knee recoveries.

It makes sense if you think about it this way: Prior to surgery, as our knee deteriorates, our bodies make adjustments to ease the pain. These changes are subtle and occur slowly, so we don't notice the extent to which we are favoring the leg or limping or splaying our foot to the side. We live with that bad alignment for so long that, when the knee is fixed and our alignment is corrected, all the surrounding tissue has to "relearn" what to do.
It takes time for the muscles and soft tissues to adapt to the new alignment of your leg, but it does get better.

One other thought: If you are limping/leaning to one side, consider going back to use a walking aid to give your body more support. You might also want to focus on heel-toe walking.

It seems to suddenly have gotten better thank you for asking. I actually stopped the patches even the Advil and started to wear new “ bouncy “ sneakers . Weird but that seemed to change everything . Other than an occasional sharp butt pain in the last 48 hrs maybe I’m at the turning point of relief!! I can even sleep!
I still haven’t walked up to my cousins place probably a 1/2 mile hike on cobblestone up from my place . Up is easy but I’m afraid of slipping on way down because of the moss. I still don’t understand how I managed the walk in my youth with clunky Candies high heeled wood sandals !

I find gel heel insoles help cushion my joints. I have them now in all my shoes and replace them periodically as they wear out. They make walking more pleasant and give a little "bounce" which my hips/ back and knees seem to like.
