Sorry I am getting on this case late. We just moved into a new clinic, TKR patients are coming in like crazy and I am leaving for the North Pole in 3 weeks.
I would like to see you if you could sit on a Ther ex ball, 65cm size and just rock back and forth. These joints are made to move and they will get sticky if they don't. The ligaments on the inside and outside of the knee need to keep their length otherwise the shorten and it will be difficult to obtain full extension and good flexion.
As far as strength ex to the leg, it is hard to keep up your strength because you are PWB and have pain. You cannot strengthen if you have pain. The body will always protect itself, this is why you have pain and you limp. You can do Straight leg raises which should not cause pain in your knee. This ex is not to improve your quad but to keep the hip flexors strong so it is easier to get into a car, bed, bath or whatever you need to pick your leg up.
I like sitting on the ball and rocking back and forth because it is more comfortable, and you can do it anywhere. You really only have to do it for 10 min a few times a day. The CPM people suggest 6hrs a day. No evidence on that method.
I feel you are lucky to have docs that are really addressing getting the bugs out. It is hard enough with a clean joint.
So, do some rocking to keep the ligaments loose and some straight leg raises to keep your hip flexors strong and you will be ahead of the game.

Thanks so much for the advice. It certainly sounds reasonable! What do you think about doing laying wall slides for ROM?
 
I have to chime in and say I used one of those exercise balls and it made flexion so easy. You can sit on it watching TV and not even really think about it much.
 
maryo52 I'm so glad to hear it worked well for you! I just ordered one from Amazon. It is expected to arrive while I am in the hospital...so perhaps I'll even ask a friend to help me get it inflated! I'm notorious for not asking for help, despite a long illness and many surgeries, I still struggle with this!
 
The wall slides are more painful. I leave for the Arctic Circle with the National Geographic Society in less than 3 weeks to measure ice melt and the polar bear population. There is great concern the bears may need food drops.

Well tomorrow is the big day!! I bet you drove down from the mountains today. I am sending lots of hugs, energy, prayers for a successful surgery. Keep us posted.

Well, today's the day! I've done everything possible to prepare, but gosh darn it I'm still nervous! :boohoo:

You are correct, Elliemae we drove down last night and oh my goodness is Denver having such amazing warm weather! I LOVE snow but I'm grateful for this reprieve!

I'm to have a PICC line placed in pre-op before the surgery. So that's the first stop of the day!

I'll talk with you all soon...and I'll be one step closer to having a functional knee!

Today is day 2 post-op and we've finally figured out pain control plan that seems to work well for me. I'm on oral Dilaudid, 4 mg, every 3 hours. Yesterday my pain was uncontrollable & I received massive amounts of IV narcotics with little relief. I finally figured out that my femoral block tubing was occluded and I hadn't been receiving the numbing medication. Duh!!! Once that was fixed I felt MUCH better!
The plan is for me to go home with the fem block in place. After yesterday's experience I'm worried about pain control once that's gone.

The surgery went well & my OS was pleasantly surprised from an infection standpoint that it looked cleaner than he expected! I originally had spinal anesthesia & a fem block. However, for some reason they added in GA. Any insight as to why they would do that, Josephine?

As far as PT goes, I'm only allowed to do heel slides, ankle pumps, quad sets, and butt check clinches...sorry, I can't remember what those are called, but it's an apt description! :) No CPM & PWB (30%) until the next surgery.

Everyone has been wonderful to me & really gone above & beyond to make me as comfortable as possible. Well, I'm going drift back to sleep and hopefully will catch up with everyone in a few hours!

Sometimes it's just what the anaesthetist prefers to do. :shrug:
 
Glad to hear things are smoothing out for you. I can understand why you'd be concerned about pain once the block is removed. But....you just need to continue to be vocal, assertive and not wait to say something if you find yourself even remotely uncomfortable. If you do that, I think you'll be just fine. It sounds like things are going just fine and I'm encouraged by your latest post!!! Get lots of rest, now....that's your job!!!
 
Hey Min!!!! Looking good on your progress and thanks for the blocked block info. I will now know what to look for if that happens to me... 4 more days, YIKES..

I'm home! :yay: The 2.5 hr drive back up to the mountains wasn't too bad. I just laid across the back seat with a pillow between my knees. Note to self for the next surgery; bring a blanket! The nurses loaded me up with Dilaudid & Compazine (anti-nausea) & increased the concentration of the Ropivicaine infusing in my Femoral Block. I'm supposed to pull out the Femoral Block sometime today...I think I shall wait until 11:59pm...that's still today, right? :what:

My local pharmacies aren't able to fill the arixtra (anti-coagulant) until Monday. Does anyone know if Lovenox is an acceptable substitute? I have 5 of the Lovenox at home left over from another knee surgery.

For those of you in the pre-op zone, here are the things that I found super helpful in the hospital:

Nivea Chapstick
Single Use Eyedrops
Mints/Gum
Dark Chocolate :)
iPad
Knit shorts/tank tops (so that IV/Picc Line is assesible)/Cardigans or Hoodies


On the catheter subject....I was greatly opposed to it, but by the time we got to talk in pre-op about it I had already been given Versed & didn't care! :beer: BUT it ended up being the best thing ever...I was way too weak & sick to worry about getting up. I asked to have it pulled on the 1st post op day & I just had to wait for PT to clear me getting up & down to go to the bathroom. They cleared me, the catheter was pulled and all was right with the world! :D

I'm just nervous because my OS is 2.5 hours away & narcotic prescriptions can't be called in. :sorry: Hopefully all goes well! I do know there are other resources available to me, I just hope not to have to use them!
 
Hopefully you'll be able to transfer to a non-narcotic pain medication like Tramadol soon. The rules aren't so strict with it and for many people it does a good job. You can also combine it with Extra Strength Tylenol for an additional boost. But....it will be a bit before you are ready to make any changes, so...just try and do the best you can.

So how are your pain meds going to be handled?? Is there a doctor (maybe your GP) there in Vail who will write the prescription for you? Or does someone have to drive the 2 1/2 hours to pick it up???? That would be horrible and really stupid!!
 
Also I think Vicodin/Narco rule isn't as strict. I could be wrong. (Me wrong? It has happened.) Anyway, not as strong as Oxy but it's pretty good stuff.
Re your anti-coagulant: Best to call the pharmacist and ask about equivalency. Not good that the hospital discharged you without understanding it's not an easy-to-get prescription.

Thanks Jamie for the reminder about Tramadol! I chatted with my OS yesterday mentioned it & he suggested alternating Tramadol & Dilaudid with the goal of gradually transitioning to Tramadol. Because of the large doses of IV antibiotics he does not want me taking acetaminophen. I started alternating the two last night & had way better pain control with less of that woozy/out of it feeling.

My OS is in Denver, however I have a local OS that I was just informed, has agreed to step in if necessary and prescribe pain meds. Other than that I'll be seeing my Denver OS every 2 weeks until the next surgery, so if we plan ahead that should be sufficient.

pdx, you are right, I was informed that they never should have discharged me without filling all the prescriptions!

Ethical question for all you smarties: My ID dr (who is new to me) marched in to my hospital room on post-op day 1 and went over the plan for antibiotics and weekly lab tests. After reviewing all of that he informed me that I must receive my weekly IV antibiotics & supplies from his infusion center. I was exhausted & not feeling well so I did not question him. After napping & thinking about it, I called his infusion nurse, and told her while I would be still be seeing the ID dr, but I would be using an infusion company that I have used every other time, that delivers to my home,and makes it super easy for me, and that I would be doing my labs at a local hospital & would have all results faxed to them immediately. The nurse agreed with me that it did not make sense for my recovery to trek 5 hours round trip weekly to pick up IV supplies & have labs drawn. Fast forward, the ID dr called me in my hospital room & proceeded to berate me over choosing a "substandard" infusion company & lab. He told me I might need to find a new doctor because he could not ensure the quality of the antibiotics & lb tests if they weren't done at his infusion center. I stood up to him & told him that these were the choices that I was making. He then stated that it was my decision & I would live with the consequences & hung up on me!

Is it not a conflict of interest for him to promote his services??? I spoke with a friend who recommended I file a complaint with the State Board. Any words of advice of this?
