THR Tendinitis after THR

I don't know how to offer a quick reply, I don't do forums...so I have to quote your message before responding, is there an easier way? I just wonder if your symptoms are recent or have been present all along? If they just recently started, it could be that you are doing too much. I think you should contact your surgeon, especially if this is a new problem for you. I've been in denial for months in the hope that my symptoms would resolve. If in doubt...seek out your surgeon. Have you been doing something new/different? Good luck!
 
@tiona The quickest way is to tag the member by putting the @ sign directly in front of their user name but don't leave any gaps eg @55beachgirl The user will then receive an alert.I agree with Tiona Beachgirl that you need to see your OS to find out what is causing your pain as it is new and bad enough to wake you at night.In the meantime it may be prudent to cut back on the activities that aggravate your hip.Keep us updated.
 
Had a CT Monday, picked up the report and DVD. Good news is that there is no evidence of loosening and alignment is appropriate. Also mentioned OA in my left hip and lumbar spine issues both of which are known to me. I didn't expect a loosening or alignment issue, so the images are the key. I see my OS today for his opinion, but in viewing the scan it is absolutely apparent that the cup is protruding significantly. I had a shallow acetabulum to begin with, which is what has lead to OA bilaterally. Femoral Acetabular Impingement. I see OS this afternoon.

@tiona yes you tagged me correctly.if your implant is in alignment and there is no evidence of loosening but the cup is protruding does that mean that a tentotomy would be the appropriate course of action ?
 
Well, there's not a lot written about it, but from what I've read a tenotomy is very successful. My OS has never done one for this particular condition, and is very hesitant to do so. He feels as if a tenotomy would relieve pain/inflammation, but contribute to weakness. I've read studies which deny weakness as a side effect. I suspect that my OS may refer me out, though I'm willing to attempt more conservative treatments first, for example a steroid injection/rest. Studies suggest that a protrusion of just 2mm can lead to tendinitis and mine is clearly way beyond 2mm. The idea of a revision freaks me out, so I won't be signing up for that anytime soon....(I don't think).
I've been told by all the docs and surgeons I work with that my symptoms are absolutely not the norm at 8 months post-op, including my OS...

Absolutely @tiona I am 10 months out and my cohort of hippies are, by and large, very active doing bootcamp exercise classes, Zumba,yoga, Pilates and hiking. I have to restrict my walking to no more than 10 minutes on level ground and so far the exercises given to me by the physio cause delayed pain.

I saw my OS's Registrar yesterday and I have been referred on to the waiting list for a cortisone injection which will take about six weeks. In the mean time I will continue with the physio. I asked him about the possibility of an impingement causing the tendinitis but he told me that the lateral view X-ray showed all was well with the new hip.

I go back on 25th March when they will review the left side which is showing signs of OA. Please let me know what your OS has to say. Thinking of you

That's because there are many muscles and ligaments around the hip joint and where one ligament is de-activated the rest will quickly take up the slack.

My self diagnosis has been confirmed. The cup is too large, my tendon is scraping against it. My OS wants me to go to an aggressive sports therapist and give it another couple of months before deciding what to do. A revision is out of the question, too many potential complications. I can choose to have a steroid injection, but that would be "just a temporary band aid." Ultimately a tendon release may be my only option. My OS does not recommend resting the tendon, as the issue will be ever present. It may feel better after a month of rest, but will rapidly worsen as I resume activity. Rest will be of no value, and even detrimental in terms of deconditioning. I'm very disappointed, but not surprised. I intend to work hard seeking solutions, and may even consider acupuncture. Keep me posted about your case. I wish you well.

A revision is out of the question, too many potential complications.
Emphasis on the word 'potential' which is the exact opposite of 'inevitable'. If the cup really is out of alignment then in the long run, you may not have a choice. As you yourself said, the alternatives are going to make things worse - a lot worse. It seems much more logical to fix the cause rather than reaching after questionable solutions. That just my 2 cents!
 
Oh dear @tiona how disappointing . I feel your pain.....I too just want my life back and to be active.

I'll follow your story with much interest as I have similar pain.....it sounds like your OS is good though...not denying there is a problem like some do

Wow ! @tiona you have been given the complete opposite advice to me.I have been told to rest but you have been told this has no value.I certainly think its true that refraining from exercise can be detrimental in terms of deconditioning but I am also scared of making myself worse.I was told that 30% of patients get total relief from the injection, a further 30% get partial relief and it doesn't work with the rest but you have been told its just a temporary solution.We just have to follow the advice of the professionals who did the surgery and hope that we both get the results we hope for.I would be very interested in the exercises and advice that the sports therapist gives you and how you find the acupuncture.
 
@Vetgirl It seems like the three of us are in the same position in just wanting our lives back and to to be active again.I feel that I am in limbo land and having to rest again is making me feel very low at times.I went through the surgery to get my life back and I almost got there last summer until this wretched tendinitis returned in the autumn.It is so frustrating.I know that you are going through the waiting game and I really hope that the antibiotics get rid of your infection very soon.Have you discussed the possibility of tendinitis with your OS.I really hope that you don't have it.Take care.We will look out for each other.
 
Hi @Josephine, The potentials are too risky for me to contemplate. Since a smaller cup is needed and my acetabulum was reamed to accommodate this larger cup a revision would require bone grafts, screws and cement, opening the door to many scary complications, extended recovery, risk of failure and the possibility that my new hip would wear out fast, not to mention infection. I need to continue to work, and have medical insurance, I simply can't take the risk of extended recovery and prolonged leave. A tendon release is something that I will consider in a couple of months if conservative treatment does not improve things. I may seek a second opinion. My medical insurance is great...IF I utilize my own providers. If I go out of network I'm looking at up to $10,000 out of pocket.

Hi @Kim22,
A CT scan is the only way to determine if your tendon is being compromised by your prosthetic. A simple X-ray doesn't give a clear picture. A look at the CT scan told the tale, one my OS could not deny. It sounds like you have to deal with a long wait for things medical in the UK. Patience is not my virtue, but I'm trying hard.

@tiona You are right when you say we have to wait a long time for our health care which is one of the drawbacks of a National Health Service but the big advantage is it is all free.Of course we pay for it in taxes throughout our working life.I need to discuss the possibility of a CT scan with the OS at my next appointment if the physio and injection don't work.Thank you for the information.I must admit that patience is not my strong point but I have had plenty of practice over the past 3 years when my hip pain first started.
 
@tiona just out of interest what time is it where you are ? I think you are in California.It is 5.20 pm in the UK.
 
@Kim22, I am in California, it is 9:20 AM here Pacific Standard Time. I figured we had an 8 hour time difference. My daughter lived in Italy for 6 months. I do try to remember how limited I was pre-hip, and to be honest even this result is better than pre-hip. My son is physically disabled. He has CP, walks with canes, has a wife, baby girl and full time employment. I think of him when I start throwing myself a self-pity party:boohoo:
 
@tiona Gosh you must be so very proud of your son and his achievements.He certainly hasn't allowed his disability to get in the way of his life.I agree with you that even with the tendinitis I am a lot better off than I was before the THR.At the moment I could walk a lot further but have drastically cut it back as advised.Walking doesn't hurt most of the time but trying to flex my leg is bad but extending it after flexion is very bad.The physio told me to draw the hip into the socket before flexing and extending and it does help but so far I can only do 3 before I am exhausted and I found by doing more than 3 I experienced more pain about a couple of hours later.The only other 2 exercises I am doing is a gentle butterfly groin stretch and clams.
