Okay2. 2014 RTHR, pain level when I am not engaging the muscle is 1, pain level when I lift straight leg from lying down is 5-6, lift knee straight up 4-5, getting into car 4-5, and stairs 3-4.
That's no good. You may as well not take it at all if you're only going to take it once a day. It's important, for pain such as you are experiencing, that you have a steady load of medication in your system throughout the day. So try taking 1,000mg Tylenol 4 times a day at 6hrly intervals.3. I am currently only taking 2x 500 mg of Tylenol before I go to bed.
Okay6. I am very active, I live out of town, my husband is recovering from rotator cuff surgery. I go up and down stairs, I walk to the barn, clean stalls as needed, I do the cooking. I drive to town and do the shopping. Working in the garden (with some help)
7. 2014 RTHR
I had more help around the house in 2014. So i did a lot more exercises.
sitting slides with rubber band in door jam (15) 1x day
knee squeezes with ball (15) 1x day
sitting clam shells with rubber band (15) 1x day
stationary bike 15 min, high seat low resistance 1x day
quad and buttock squeezes (15 each) 1x day
I walked a lot, approximately a mile or more several times a week.
2018 LTHR
I did not have as much help this time, so I did/do fewer exercises, I walk more, and I try to stretch more.
at the end of two weeks post op I was doing the following:
knee squeezes with ball (15) 1x day
sitting clam shells with rubber band (15) 1x day
stationary bike 10 min, high seat low resistance 1x day
quad and buttock squeezes (15 each) 1x day
At almost 6 weeks If I do the exercises I do the following:
knee squeezes with ball (15) 1x day
sitting clam shells with rubber band (15) 1x day
gentle squats holding something (15) 1x day
Elliptical 20-30 min 1x day
Apart from the medication, the only thing I see as a problem is that you are doing far too much and have been doing far too much for ages! Obviously you failed to spot this bit of advise which I think is key to your entire problemPT:

Currently I go once a week,I hop on the elliptical for 20-30 min, and then we work on the hip flexor, deep tissue massage, cupping, and dry needling
Hips actually don't need any exercise to get better. They do a pretty good job of it all on their own if given half a chance. Trouble is, people don't give them a chance and end up with all sorts of aches and pains and sore spots. All they need is the best therapy which is walking and even then not to excess.

, particularly for hippies! I got on from the ground for years, but as the horses got taller, and the research showed it wasn't great to get on from the ground because of the pull on their backs (and who knows what it was doing to my hips@), I started getting on from the fence with my really big chestnut. I started using a mounting block when my hips started to hurt, to get off on instead of jumping down. I like to teach them to siddle up to the trailer wheel hub or anything else that I can get off or on from. Anyhow, I've taught my young guy to line up either way by the mounting block, and also I taught him to come to the fence either way. I'd drape myself over his back and give him scratches, but never got on as I was so tight in the hips.
. The rehab takes so blessed long, it is frustrating!!!
. I feel I have to be quite overall fit to ride my horse again....does that mean putting off riding for a little longer than three months, to get that fitness going, I don't know and I won't know how one feels at three months, until I get there? You have that advantage. My question is, how does one get that fitness back with out getting so darn tired! I am really starting to thinking about pool walking, although I can't stand the smell of chlorine (being from Bermuda I am quite spoilt!) and yoga....although I have started the pilates 100 again for my abs!! Thinking out loud here, perhaps a better way of thinking in what to trade off....the barn chores require a lot of walking, but what do we need most to ride....in my discipline it is really strong abs, as they help so much with balance and all that. To me I need to stretch out the muscles in my legs, and realign them as well. Correct walking helps with that. I am finding the same thing, I over do it constantly, not enough to put me in the ODIC, but enough for my poor muscles to say, "could you please prioritize, and not try to do everything!"...and that is just with activities of daily living. I feel your struggle, although I'll be put right back in my place with the second OR! Thinking of you on the muscles issues and getting back on! Will be watching your journey
