Revised PKR to TKR Patellofemoral PKR with Ehlers Danlos hypermobility

You should be able to sleep on your side in a few weeks. I started side sleeping by laying on my non-operated side and keeping my leg pretty straight. I then put a pillow on the bed above my old knee and put my new knee on the pillow. I could sleep for a few hours this way and then go back on my back when my knee started to hurt.

After my first tkr, at my first visit to my massage therapist, she had me sidelying on my unaffected side with my postop leg fully on a bolster as high as my natural hip level. This made it so easy to flex or straighten that new knee as much/little as I wanted.
So with my second tkr, I added more pillows in order to side lie.

A little progress! I can sleep a little longer on my side propped by pillows. My MRI for possible sciatica is tomorrow-hope it’s a fast one as lying in the machine without a knee prop should be interesting!

Have you asked your doctor about Gabapentin? It relieves my pain about 25%. That's better than nothing
I also got some benefit from gabapentin. It wasn't a total solution by any means but it did help a bit.
This seems to be not that uncommon in the recovery period. A lot of us have spinal arthritis as well as knee arthritis and it can flare up in the recovery period due to imbalances or less walking.
I found that once I could get on the floor to do back exercises it did improve- but in the early stages it was miserable.
Once I was off opiates I persuaded my GP to give me some zopiclone sleep medication. A decent sleep made all the difference.

I've had MRIs and CAT scans and they always allowed me to put a rolled up towel under my knees, for comfort's sake (and to keep me from squirming I suspect).

The hardest part for me is not falling asleep inside the tube - I'm not claustrophobic at all.

hope the MRI isn't too uncomfortable. Let us know how it goes.
Hi-thankfully, lying in the MRI yesterday wasn’t overly uncomfortable. I have the radiology report but haven’t met with my doctor yet to see what it means. Any insights into the following are welcome:

There is mild stepwise retrolisthesis at L2-L5. There is trace anterolisthesis at T11-T12. There is slight lumbar levocurvature. The vertebral body heights are maintained. There is mild left L4-L5 Modic type I endplate marrow signal. There are scattered Schmorl's nodes. The conus terminates normally at L1 and demonstrates normal signal. There is a small hemangioma within the T11 vertebral body. There are sacral Tarlov cysts at the S2 level measuring up to 1.6 cm in AP dimension on the left.

Level by level:

The last well-formed disc is designated as L5-S1 for the purposes of this report.

T11-T12: There is a small central disc protrusion on sagittal imaging with minimal spinal canal narrowing.

T12-L1: There is a central disc extrusion with annular fissure which measures 3 mm in AP dimension and extends 1 cm above and 4 mm below the disc space with minimal spinal canal narrowing. There is no significant neural foraminal narrowing.

L1-L2: There is no significant disc bulge, spinal canal, or neural foraminal narrowing.

L2-L3: There is a broad-based 2 mm disc bulge without significant spinal canal narrowing. There is bilateral facet arthropathy with small bilateral facet joint effusions and moderate bilateral neural foraminal narrowing.

L3-L4: There is a broad-based disc bulge measuring 2 to 3 mm in AP dimension which together with ligamentum flavum hypertrophy, bilateral facet arthropathy, and epidural lipomatosis results in mild spinal canal narrowing. There are bilateral facet joint effusions. There is moderate to severe bilateral neural foraminal narrowing.

L4-L5: There is a left paracentral disc protrusion measuring approximately 3 mm in AP dimension which contacts the descending left L5 nerve root and together with ligamentum flavum hypertrophy and bilateral facet arthropathy results in mild spinal canal narrowing. There is severe bilateral neural foraminal narrowing.

L5-S1: There is a central disc protrusion measuring 2 mm in AP dimension without significant spinal canal narrowing. There is facet arthropathy with moderate bilateral neural foraminal narrowing.

IMPRESSION:

1. Advanced bilateral L3-L4 and L4-L5 neural foraminal narrowing.
2. Moderate bilateral L2-L3 and L5-S1 neural foraminal narrowing.
3. Multifactorial mild spinal canal narrowing at L3-L4 as detailed.
4. Left paracentral disc protrusion at L4-L5 as detailed contacts the descending left L5 nerve root with mild spinal canal narrowing.
5. Left L4-L5 Modic type I marrow signal, which can be a pain generator.
6. Multilevel listheses as detailed. Annular fissure at T12-L1.

Glad you have this behind you. Spines are a complex issue and we don’t really have anyone on our BoneSmart staff who can accurately interpret this report. So…..we’ll be interested to hear what the experts have to say. Hopefully you’ll know that soon.

I am no expert but I have had similar MRI findings. I think the gist is that the changes at L3-4 and L4-5 are catching the sciatic nerve and causing the pain. I am not surprised you are having problems.
I was told that if mine did not settle with straightening my leg/gabapentin- then it looked like a steroid jab would help.
Thankfully mine did settle and now is OKish with just regular back exercises/ pilates. I only get twinges if sciatica on long car journeys.
I hope your doctor can help you to find a solution for the pain.

This afternoon I am off to a spine pain specialist (anesthesiologist too) for an appointment and possible injection. Will see! My Dad has gone home so my activity has picked up exponentially—aggravating the low back and hips.

On a related note, does anyone have recommendations for a good mattress topper for side sleepers under $100? I like a thick pad that doesn’t get hot.

Sorry you're struggling with pain. If you receive an injection, I hope it provides speedy relief.

Re: mattress toppers,
IF you're an Amazon shopper, many aren't, consider checking there. I look for name brands, "best sellers", ratings by stars 1-5, and I read reviews. I also check out the photos posted by those that have purchased the item. Foolproof? No...but more times than not, worth the effort in my opinion.
Good luck, Mochapup. If you find a good one anywhere, let us know.
A great weekend to you!
