THR Worsening pain

New to the site, thanks for accepting me.

I am a 76 year old female and 2 years ago I had a spontanious crack to my sacrum which developed into two cracks. The ortho specialist arranged for me to have a prodedure to inject 'cement' into the cracks which worked well.
Unfortunately in July 2021 I fell and broke my right hip. The surgeon decided to do a complete hip replacement which went very well. Within 3 months I was walking with a slight limp and very little pain. from January this year I was starting to have a lot of pain in the back of my thigh especially when I sit. I can hardly walk when I stand up after sitting. I went back to the orthopedic specialist and had xrays and an MRI but they could not find anything wrong with the implant. It seems to be getting worse and I really dont know what to do. Has anyone else had this problem?

I have not seen someone with your history of sacral cracks and later a THR after a fall.

Sounds like you may have soft involvement with your pain, did your MRI report anything about soft tissue injuries?
With your history of a spontaneous crack, has your surgeon checked for occult cracks? A bone scan would reveal areas of increased metabolic activity.

You may want to seek a second opinion from a surgeon specializing in revision or complex joint reconstruction, they have the training and experience to get to the bottom of what is causing your pain.

Hi @Lynno does the pain feel like you are sitting on it? There is this thing called Piriformis Syndrome, which is a huge pain in the behind from one little muscle. There's an article in the Bonesmart library about it.

My rear-thigh pain is almost all from a malfunctioning sacroiliac joint, but with this, the joint itself hurts, and sends pain up to my waist or above, and down as far as my feet. I have had piriformis syndrome, one time for about 5 years, but didn't know to go to a doctor, if you can imagine that. The pain was awful, and my job as an orchestral musician required me to sit virtually motionless for a couple of hours, a couple of times a day. And it hits me almost every time we get in the car for a long (10+hour) drive. Go figure.

I'm sorry you're having this, after such a good recovery of back and hip!
 
Still in lots of pain and have been started on LDN (low dose naproxalone) heard tons of good reports of this medication helping with chronic pain. Any input would be appreciated.
