Update on TKR 10/31/2023 & MUA 4/2024:
Had Knee ablation in Nov 2024. Didn't help.
Physiatrist did needle procedure ( not ablation) along ilio- tibial band. I feel much better on outside of thigh. I don't feel pulling.
But, I still have the same knee pain from post- surgery 10/31/
It actually feels worse.
Entire knee running east-,west ( I don't know medical term) is very painful when I walk a few blocks.
It's extremely painful when I sleep .
Gabapentin, Voltaren, Lidocaine , etc does not help.
I haven't had physical therapy because my sessions ran out for the year on my health insurance ( I can start in January)
I have been massaging and icing affected areas.
I think there's something wrong
Should I go back and see my original orthopedist who performed the TKR and get a new MRI and x-ray.?
What is causing the increase in pain? Is my knee so weak? Is the IT band still pulling on the tendons?
I don't know what's happening.
Suggestions, input would be helpful please.

An appointment with your surgeon is a good idea. We have no way to assess all the possibilities - yes it might be soft tissue, yes it might be nerve, yes it could be orthopedic.
If after seeing him you're not satisfied with his work up and plan, you could consider a second opinion.

If you do, we suggest you seek that second opinion surgeon from a practice and a medical center completely independent from your original surgeon. This isn't because we think anything went wrong in surgery! The problem we have seen is a lot of orthopedic surgeons tend to look at the x-ray, tell us we're actually fine, and that's that.

Second, we suggest the second opinion be from a surgeon who specializes in knee replacement revision. This isn't because we think you need more surgery! It's because these are the surgeons who see lots and lots of "problem cases" and are better able to diagnose your knee problem.

I agree with your assessment. I did try to make an appointment awhile ago, but the doctor said I had to wait two years post TKR. So now I can make that appointment.
I have other medical hospital centers to choose from in my area. I live in NYC.
You have given me a way to move forward in the new year.

MUA: 4/2/24

Happy New Year 2025.It's been a while.

I had knee ablation which didn't work. (December 2024) and an injection in Jan 2025, which helped a little.

I'm back in PT (I had to stop last year because my health plan ran our of sessions)

My main issues now are:
1) Stabbing pain around top of knee area and circling around there - no change since TKR
2) Pain all along Ilio-tibial band at specific trigger points since post MUA - (My fault due to over-exercising on leg-press machine)
3) I can't walk more than a few blocks without extreme pain at front of knee and side of leg)
4) Physical Therapy massages & stretches haven't seemed to help much (+TENS)
I don't mind the stretching- it feels good. The massages on the the trigger points are painful, but that's where the tightness is.

I don't easy stretching at home. Ice my knee.
I can ride stationary bicycle at level 1 setting without pain.
I can use rowing machine at low resistance without pain.
I have new Orthotics to help balance my feet & legs to compensate for one leg shorter then the other (which is good for my hips & balance) -- but I still have same knee + IT band pain.

I don't know what more I can do.

Have you considered just stopping all exercises and doing just your normal daily activities? Try this for a month and I believe you'll have less pain. Your muscles and tendons have been traumatized since Halloween of 2023 and have not had the chance to heal from that trauma. What you have been doing hasn't been helping, so why not try something different? It can't hurt.
 
Thanks for your advice. I'll have to think about it. But I still need to ice my knee. I'm just concerned about leg weakness.

I don't know what more I can do.
As sistersinhim suggested, you're likely overdoing it, so there's probably nothing you can do to improve your situation aside from stopping your current level of activity which seems to be aggravating your knee. If her suggestion of stepping away from the exercises for a month feels overwhelming to you, how about trying a shorter timeframe, let's say two weeks, and see if you begin to notice a difference.

It's been a long while since the surgery and if you've been pushing it like this the entire time, It's not surprising that you're dealing with knee and IT Band pain. Please consider giving yourself some TLC in the form of the break your body needs to recover.
