Dear All members,

I had late onset infection on lumbar spine's implant 8 months post-op. At about 5 months post-op, I had fever for a week then it went away, night sweat......I knew something wasn't right but the last thing that come to my mind was infection. Like most patient, I thought it's just another setback from major surgery.

Finally, MRI confirms enormous abscess (Bio-film) at L5-S1 implant. The bacteria was never found in the blood although CRP was between 8-12 at that time. Surgeon washed out the abscess, removed the infected implant and replace with another implant made of pure titanium.

The bacteria identified as Staphylococcus Aureus (MSSA). At hospital on IV for 4 weeks, followed by daily oral antibiotic Levofloxacin 750mg and Rifampin 600mg. 3 months later, ID doctor switched me to Doxycycline 200mg and Rifampin 600 mg. I have been on oral antibiotic for total of 9 months to the present

Below's the timeline for infection post-op. After the wash out, all my monthly blood work is back to normal, CRP<5. Nothing shows on blood culture either

-2.5 weeks post-op, MRI shows significant smaller collection/inflammation at surgical site

-8 weeks post-op, MRI did not shows finding to suggest organized abscess, osteomyelitis or cellulitis

-3 months post-op, Bone scintigraphy(Tc-99m) and Gallium scintigraphy (Gallium 67): no evidence of residual infection

-9 months post-op, MRI shows no sign of infections

I have gotten a second opinion from the ID doctor outside ........the infectious diseases doctor i consulted have vastly different opinion/ treatment plan

1. Original ID Doctor suggests to stop all antibiotics, said I have a high probability of being cured, continue blood work monthly and MRI every 3 months, if nothing shows after 18 months, you are cured.

2. Doctor B said continue Doxycycline as long as I can tolerate, eg. few years or even lifetime. She said Staphylococcus Aureus is very hard to eradicate 100%, relapse is high.

If the infection returns, I am afraid I will need to undergo another spinal surgery. But taking antibiotics long terms does concern me, I am 40 years old. Any suggestion, comment is much appreciated.......

Questions

1. I live in Dallas TX, does anyone know any good experience ID doctor in this area? RestAssured? I read your one stage revision was done in Plano TX. Can you recommend your ID doctor who work with Dr. Gerhard Maale?

2. Anyone here had successful one stage revision hip/knee?

Regards
Michael

I'm a bit confused, Michael. You posted in the hip recovery forum, but nothing in your post leads me to believe that you have had or are in need of a hip replacement. Is this the case and you're just looking for information about @RestAssured 's infectious disease doctor for another opinion about your spine?

Although my infection is from my spinal implant, there are similarities in the treatment. I guess mine is somewhat like a 1-stage revision.

1) I would like to find out how long members in this forum who had an infection were (or will be) on oral antibiotics. I'm afraid to stop and risk a relapse but I'm also very afraid of the side effects from long term antibiotics.

2) Since RestAssured mentioned her doctor is in Plano TX, I thought I'd get another opinion from an ID doctor.

3) Is it true that the only way to find out if the infection is completely gone is by stopping antibiotics and wait? So far, all my blood tests, MRIs, and nuclear medicine scans have come back negative for infection. But I was told that they're not 100% accurate.

@kflee00 type: infection in the search bar at the top of the page to bring up members' infection experiences which may help you.

Have heard amazing things about Dr Maale in Plano, why not call his office and check it out?

@Horseshoe: Thanks, I have spent the last two weeks reading member who's had gone through infection treatment and agree that this is going to be a long and lonely journey,:elevate:

@kflee00 Dr Maale is world renown! I would suggest you call his office and get his opinion. I believe he does consultations via Skype and/or phone if you can't travel.

You have already tagged RestAssured. I am sure she will be around soon.

Why wait for RestAssured? She may not be online for a while and might not see your tag. Just call Dr. Maale's office and ask for the names of the infectious disease doctors they work with.

Yes, two or three people have given you her surgeon's name. He's a very nice chap. I met him once when he was visiting the UK.

Hello @kflee. I have been most fortunate to have had Dr. Gerhard Maale do my revision. It has been over 5 years and my knee is still doing fantastic! [emoji2]

Hi @RestAssured

I'm glad to hear that your knee isn't giving you problems anymore. Can you please give me the name of your ID doctor? How long were you on oral antibiotics?

My ID doctor that I had in Dallas passed away. I now have Dr. Povis in San Angelo TX and she is wonderful! She and I have discussed going off the low dose of Bactrim I am on when my other health problems clear up

I think the ID doctor Maale works w now is Dr. Allen. Call Maale's office and they will be happy to help you.[emoji4]

Hi, @RestAssured

Thank you so much for the ID doctor info. Looking at your timeline, I am guessing you've been on Bactrim for 5 years?? I sincerely hope you can get off Bactrim in the near future.

If anyone knows the answer, please comment. @keepmovin, @KarriB, @RestAssured

My ID does run labs that can tell if your body is fighting an infection, but it's not definitive to where the infection may be. After my IV antibiotic and oral rifampin for 6 weeks I was off all antibiotics for 2 weeks because my white count dropped alarmingly low. Once it came back up the ID prescribed 100mg of doxycycline 2x a day. After a year it went down to 100mg once a day. This has been 3 years now and I believe as long as my labs are good I'll stay on it, my ID prepared me by saying possibly for life at the beginning. I wasn't happy, but when I thought of the alternative at that moment I got over it in a week or so.

My only noticeable side effect is a loose bm. I do take a good probiotic because C-diff can be a nasty side effect. I've never had a nuclear medicine scan or an MRI for my TKR.

@kflee00

We cut down on Bactrim from 2 pills a day to 1. With all the incidents of MRSA I had, we both agreed that 1 a day was ok. My ID doctor has talked about stopping it in the next year.

I have blood tests run every 3 months and all have been good for 5 years
