Also, perhaps a second opinion? With a doc who is used to doing revisions and thus is a person who hunts for problems in the knee?

I am starting to think it is something to do with the knee joint itself.
Most unlikely!
Was told by one doc that it was related to IT band tightness so have been doing stretching and PT with no relief.
Okay, first things first - can you indicate the site of your pain using this chart?
