Revision TKR Poly plate exchange

Still using a cane when out and at school. I went back part time first two weeks and am on
Christmas break now until 1/5. Still not sure why I can’t get away from the cane completely.
Surgeon said PT needed to “ramp it up some.”
Just waiting to get rid of the cane:tantrum2:

I really don't understand why there's a rush to get off the cane.
Your PT says your knee is gradually getting stronger, so the day will come when you no longer need the cane. Until then, why rush things and risk doing damage?.

You're only 3.5 months into a recovery that takes a full year. Your knee still has plenty of time.

It has been 2 years since I've been here. I had a total left knee replacement in 2017.
2 weeks out from a revision. The poly plate was loose and the wrong size. Curious,
has anyone had that problem? He said about 3 weeks recovery. I'm not sure that's going to happen! Things are progressing much faster than my total but not my pain! Taking tramadal every 4 hours and supplementing with Advil in between. Take 1 Percocet (5 mg) at bedtime. Took Percocet first week every 4-5 hours. Pain level is still not manageable with the tramadal. I remember the Bone Smart advice of taking meds with tramadal with my last surgery.
I can't find it. Can you help me with that? Thanks! I also don't know how to connect to my previous thread from 2 years ago.

@Lynne Cleveland
You'll notice that I have merged your two threads together as we prefer that members in recovery only have one thread for a knee's recovery. So, as you requested, you now have just one thread for your left knee.

This is why we prefer just one thread:
1. if you keep starting new threads, you miss the posts others have left you in the old threads
2. it often ends up that information is unnecessarily repeated
3. it's best if we can keep all your recovery story in one place so it's easily accessed if we need to advise you.

Please keep all your questions and updates on this thread. If you would like a new thread title just post what you would like it to be and we'll change it for you

I'm sorry that you've had to have yet another revision to a TKR. That's really bad luck.
Has the same surgeon done them all?

Please will you tell us the full date of this latest revision, so we can add the day to your signature?

I think you'll get better pain relief if you cut out the Advil and use Tylenol instead.

Here are the charts that you were looking for. The first is for 4-hourly doses and the second is for 6-hourly.

The most effective way to take Tylenol is 2 x 500 mg tablets 6-hourly, to a total of 4,000 mg (4 doses) in 24 hours. You need to take it regularly, to keep up the levels in your bloodstream. If you just take the odd dose now and then, it's far less effective.

Check all other medications you're taking, to make sure there is no Tylenol/Acetaminophen/Paracetamol in them. If there is, scale back one or two of your regular doses, so you stay within that safe 24 hour limit of 4,000 mg.

Yes, thank you for merging the threads. Thank you, this is what I was looking for. The revision was
11/21/19- poly plate exchange. It’s only been two years that I had my TKR.

Yes, it does sound a bit early. It shouldn't take as long as a new TKR or a full revision, but your knee was still cut open and will have to heal.

Take as long as you need.

I agree that it sounds early. Listen and watch your knee. If the pain or swelling increases then you're doing too much and need to rest more.

Thank you, just trying to figure out my work schedule. I work in the school system and had planned on returning next week. I haven't even been out of the house except to walk around in the yard. There are 4 days next week before Christmas break. Being a para assistant, I am on my feet most of the day in the classroom.

10 weeks post op, revision was done. Switched out poly plate insert. Was doing better and 2 weeks ago suddenly swelling in certain areas and pain level went up. Finally got in to surgeon only to be told he wasn’t going to drain it, didn’t want to risk infection and it would get better. Ask if I was icing it. Really?? After 2 knee replacements with him and 2 revisions. I told him icing was helping with pain but no change in the swelling, that’s why I was there to be seen. Told to watch for signs of infection but he “didn’t think” it was infected. Would not draw fluid to check for infection either! Asking my neighbor, who is a physical therapist to take a look at it this week and see if he can work out some of the fluid. I am back to being woken up at night with pain. I’m concerned.....

I am so sorry you are having these problems. It seems sometimes that we are going along just fine, then a wrench is thrown into the works.

Have you increased your activities? That could explain the onset of pain and swelling. Are you elevating toes above nose?

I had a revision in 11/19. The poly plate had to be exchanged to a different size. According to the op report he went through 3 different sizes. I have still having trouble! Back to a challenge when I get up from sitting for a length of time, stairs, and the pain is there to some degree each day. After contacting him,
A bone scan has been ordered. I’m not sure this is going to show anything since “apparently” it didn’t last time. He waited almost a year after the replacement telling me the bone scan was inclusive, until I went for a second opinion only to be told after examination it was a poly plate problem. Any others that have had a poly plate exchange?

@Lynne Cleveland you will notice that I have again merged your two threads together as we prefer that members in recovery only have one thread.

This is for three reasons:
1. if you keep starting new threads, you miss the posts others have left you in the old threads
2. it often ends up that information is unnecessarily repeated
3. it's best if we can keep all your recovery story in one place so it's easily accessed if we need to advise you.

Please keep all your questions and updates on this thread. If you would like a new thread title just give a shout.

I can't remember anyone on our board having this problem. But let me tag @Jamie as she has much more experience on the knee side.

Sounds like you need to work with another surgeon. Would you want to work with the second opinion consultant?

Hi, @Lynne Cleveland .... although not extremely common, it is possible to need a spacer exchange on a knee. A number of our BoneSmarties have had that procedure and have done fine afterwards. However, if you are experiencing significant pain, and especially if the pain has or is increasing, it's time to get things checked out.

Since you seem to be a bit at odds with this surgeon, I'll agree with Jaycey that it's time for some additional opinions from an experienced revision specialist. Are you in the Cleveland area? I can help you find some options for a consultation if you like. You would just need to tell me what surgeons you have seen to date.

I had a revision on my left knee 11/17. Took out a replacement and inserted another. As many surgeries as I’ve had, you would think I would have all the answers. Each one has been different.
I’m struggling with pain control.

Given Percoset 5 mg/ Acetaminophen 325 after left the hospital to take 1 every 4-6 hours. Also given Visteril to take with it.
He said it enhances how the pain meds works. It
was doing fine. At 3 1/2 weeks ran out and they gave me Hydrocodone 7.5 acetaminophen 325, take 1 every 6-8 hours. It is not working for me!

Really set me back. I called and his PA said I could take 1 every 4-6 hours if need too. I can barely get to 4 hours. Also caused nausea. Gave me Zofran for that, working okay.

Looking at the script it says under Drug name- “Monograph” assuming this is generic. Looking it up it could be Loratab, Vicodin or Norco. Has anyone taken this with little relief?

I don't think that's a drug name. It's a schedule. Can you phone your pharmacist and find out what the name of the drug is and also the dose and how often to take it?

The name above states Hydrocodone 7.5/acetaminophen 325- 1 every 6-8 hours then under that it states Common brand - See Monograph

I’m so sorry to read that you are looking for another revision specialist. :console2:

Which knee is it and what’s going on with it?
