Hi all,
I hope everyone has done ok with all these storm systems coming in.
There has been so much going on. I am doing well with my knee. Week before last at PT I got to 120 ROM. I was soooo proud! My extension was at
0. Later that afternoon I had an appointment with my OS. Well, when I got there, I saw his PA instead. It was NOT a good visit! First let me back up a minute.
I have had 2 surgeries on my right heel for removal of bonespurs. Since the last surgery (4/4/08) I have had this HUGE knot on the inside of my heel, just under my ankle. It HURTS to walk, in fact it is affecting my gait pattern.
I don't have that smooth heel to toe glide as it hurts too much. Now, back to the visit with the PA. He comes rolling into the room, puts my x-ray up, tells me it looks real good and to get up on the table. I had given my re-eval from my therapist to the nurse when I first went in the room. 120 flexion 0 extension. The PA has me just pull my leg up to a "resting position", then he tells me " I know you are not warmed up", and measures me at 110.
Fine, I understand that, then he looks at my extension, does not measure it, and asks me "are they pushing on your leg?" Yes sir,, "tell them to push harder". He then proceeds to show me a exercise for better extension, and tells me he wants me to be able to "lock that knee back".
He then tells me he wants me in an extension brace and tells the nurse to write on the re-eval that he wants AGGRESSIVE EXTENSION EXERCISES done. I told him that just that morning I was 120 ROM and 0 extension in fact I have been at either 1 degree lag or at 0 with my extension since the surgery. He just ignored me. I tried to ask him about my foot, and I guess he did not hear me, as he walked out of the room. I was dumbfounded!
Well, as I am walking out of the room my OS sees me in the hallway. His words were "wow, look at how good you are walking!". I told him if my foot were not hurting so badly I would be walking better. He asked me about it,
and I made an appointment for the next week for him to look at it. The PA stood in the hallway with us for the entire conversation. All of a sudden he just blurts out to my OS, " her extension is terrible, I want her to have a brace"etc.... My OS looks at my leg and says "it looks good to me".
At that point the PA grabs my paperwork out of my hand and takes it to the front desk and tells the girl, she needs to come back in 6 weeks to see me.
Now my PT had told me earlier that morning, that he would probably be releasing me within a couple of weeks as I was doing so well. Walking without any assistance etc..... I did not say anything to my OS about how rude his PA was to me as we were in the hallway and patients were coming and going. That was not the time nor the place. When I got in my truck to go home, I happened to pick up my eval to take back to the PT and I saw where the PA had written across the eval.... 10 DEGREE LAG EXTENSION 110 DEGREES FLEXION - UNACCEPTABLE!!!!! It was at that point that I started crying.
I felt like he took everything I had worked so hard to achieve and just threw it out the window! I went straight to my PT, boy was he ticked off!!!
He told me there was no way my ext. was at a 10 degree lag. The PA did not
even have me try to pull my leg back at all. I felt like all my work was for nothing!!! The manager of the therapy clinic talked with the OS later that night at a football game and told him that I had a bad experience with the PA. So when I saw my OS last week (hubby went with me), he told me
that he had talked with the manager about me. I just said that I REALLY REALLY did NOT want to ever see his PA again. That he hurt my feelings and was rude. I let him know why I did not say anything to him while I spoke with him in the hallway. He told me no problem you will see only me from now on.
I noticed the PA kept walking up and down the hallway before the OS shut the door. Needless to say I am NOT getting that brace, in fact my PT has been decreased to 2 times a week, for 6 more weeks as the OS wants me at a "cold resting bend" of 115 -120 degrees. He is sending me to a Ortho doc who specializes in just the foot and ankle. He seems to think my achilles tendon has shrunk and that is what is causing the pain when I walk. He thinks I made need orthotic shoes. He has also ordered PT for my foot. I am so sorry this is so long. I have gotten over that episode, and I don't intend to let that little tiddlywink get me down! I have worked so hard to get to where I am.
My PT was so glad to see that I was in better spirits the next time I came in for therapy. I felt so bad about crying, but it just took me by surprise. Here I had thought I was doing so well. According to the ones who really matter,
my OS, and my PT I am! Again sorry for the novel. You guys have always been so encouraging and friendly. Thank you for all your help and support!
I could not have gone thru this without all of you!
 
Talk to your surgeon. The PA may have other issues and the MD needs to be aware of how his PA making patient(s) feel. You need YOUR team to be on YOUR side, emotionally and professionally. You will actually be doing other patients a favor. You are well within your rights to request that you not be seen by that particular PA again. That act alone will speak volumes.
 
I'm so glad that you spoke up. The doctor responded correctly and I can't imagine that he didn't take that PA to the woodshed for a chat. Be proud of what you have accomplished!!! You have done a marvelous job and are continuing to improve!!!

Holy Moley - Time to have a lynching! I think we all head down to you r OS' office and take the PA and hang him by his --- well... you get the picture - right! How dare he! You have done sooooooo well and I am sooooo jealous of your excellent progress - I am very glad you have been able to re-connect with the OS and get things straightened out. I really like the rapport my PT and surgeon have. They fax right before I go to see the OS and then I bring the paperwork back. How dare the PA say anything is unacceptable. How would he like to have someone pushing aginst his un-moving knee. I stillthink we should go and hog tie him.

Hang in there with your foot - Sounds like you are on the right course now to get that all straightened out!

I am so proud of you for speaking up to your OS and PT about that insane PA!

So sorry you had to go through all that.
I am very glad you spoke to your OS and got rid of that PA from your life. Think of all the other patients you may be saving from his poor treatment.

What a clutz! He gets 2/10 from me for his bedside manner, that's for sure!

I am proud of you! You exerted your right to refuse to see that PA again. The PA needs to work with the patients, not against them. I bet he will not last long with this OS. This seemed like normal procedure for him.
I am proud of you too for working so hard and doing so well with the pain radiating from your foot. Seeing a person who specializes in the foot and ankle is good. It seems your OS has your best interest at heart. Do what they say and you will soon be 100% and back to living a normal life.

That PA definitely should not be in medicine!! Seems to have a strong opinion of himself and not be willing to listen or pay attention to what is right in front of him! It's tough enough to recover from the surgery without having to deal with such totally unprofessional stinkpots as he is. Seems to me that you are doing very, very well and I thank my lucky stars that I haven't run into such medical personnel in my journey. Your numbers look good to me - continue to work with your doc and PT and hope that you never even catch sight of that PA again!!

When I was going into the hospital for by BTKR you send me a PM that really pick me up. This shows what your parents raise and should be very proud of. You are what I call good people! God knows we need more like you. Now and then we run into people in positions they should not be in, like the PA you incounter. Feel sorry for him for he is missing out on being part of the human race, and pray something will bring him around.

Hey Carolinagirl, I too have a very low opinion of the PA bunch. Every time I saw the one I had for my Oxford pain she did nothing but write new scrips for more med. If she had once ordered an x ray I may not have had to have the TKR!! I'm six days out from the TKR and still in serious pain with all the usual issues.

I try to treat people the way I want to be treated, but that man really
tested my "bringing up" that day! I saw him today in the hospital, I honestly don't think he even recognized me, which is fine with me. My OS has a nurse
assistant that is the sweetest thing. She is really on the ball! I had to call her today about something and she let me know that my OS had "taken care of the situation". I am assuming there was a "dressing down" involved somewhere. Guess he needed to be brought back down a notch or two!
I am gonna get off subject here for a minute, are any of ya'll noticing a sudden rush to the gas stations? It has gotten so crazy here within the last hour! We have a scanner and it seems that all the gas stations in our town has cars lined up out into the street waiting to get gas. It was mentioned that gas was rumored to go up $1.50 per gallon overnight. On the way home from work, EVERY gas station we passed was PACKED!! They have even report one fight at a station over gas! Seems that this is all a result of IKE
about to hit Houston. Price right now is $3.65 per gallon.
Now back to knee business. I hope all of you have a good evening and are able to get a good nights sleep!
 
I was so aggravated when I read your post I had to wait a day to respond. It is so aggravating when medical support people are rude, arrogant, or incompetent. I understand when they lose it sometimes with rude patients, but it is unconscionable when a receptionist takes it upon him/herself to decide how badly a patient needs to be seen. Good PA's are a blessing, but the bad ones can do a lot of damage. If you hadn't known better, you could have just taken what was dished out, or stopped trying. I'm glad the OA proved himself by aligning himself with the "right" party in this circumstance.
