I just had my staples removed yesterday 17 days post op and my doctor gave me some insight to the procedure. I have the top half of a Zimmer next gen and the bottom is a compilation of Zimmer parts to overcome the problems associated with the failed oxford partial and the resulting stress fractures of the tibia. The area under the oxford tibial implant was crushed and had to be removed completely. That section of my tibia has been replaced with a new material that will allow the bone to regrow and absorb this new material. Wish I could remember the name it is something that he is specializing in as a revision specialist. The tibia stem is 100 mm long and had to be that long due to al the damage.
My current ROM is about 90 degrees and he wants me at 120 degrees in one month or he will do manipulation. He told me that six weeks from surgery is the optimum time for manipulation and that it gets less succesful after that time. I got some work to do!!!!

Do you mean the TrabecularMetalâ„¢ Technology, GM?

And I think the Segmental System prosthesis is what you have. Pretty good unit as I recall. Hope you manage to meet to quota!

I think gaining ROM is not something you can put on a time line. I am about a week shy of 4 months and have gotten the right one to 115 and the left about 117, with the left one having an easier of time to get that range (ie less pushing by the PT). I was stuck at 104/107 for a long time and at 2 months out the OS was considering a manipulation but given that we had a minor tibial break when he put in the right one he really wanted to avoid MUA and told the PT to get more aggressive. Well between the PT and alot of home work we started making progress. I do hope we make the 120 over the next 6 weeks before I see him again but if we don't I am happy were I am at as that is 20 degrees more than preop.

Thanks for the replies I am very concerned about the manipulation after having broken the femur b4 and having the stress fractures in the tibia recently. I thought the timeline was aggressive and maybe it is just a scare tactic on the part of the OS.

I have a feeling timelines are specific to each individual surgeon. We talked more about MUA today at my latest apt with the surgeon. I am at a bit more than 8 weeks. He would prefer me to get the ROM on my own - he feels that MUA can be beneficial but it could also cause a loss. The ROM - if achieved on your own- is more likely to remain. And I am an odd one because of swelling - an MUA could be a swelling nightmare for me. We are giving the JAS system a 2 week trial and if the progress soes not suite the partner (my OS is off to Buhtan for a month!!!) he may opt for the MUA at that time - My guy felt that we would be at a point (10 - 12 weeks) where we will need to move. I might be able to putter through until he gets back - but then it would be a done deal if I am not at 120-130. He looked at my non-operated leg and said he is not sure if we can get that (I can lay on my stomach and grab my ankle and really bend that one to the max without issues) but he would like me to get as close to 130 as possible since I am young and "healthy".

Hang in there - they are the pros when it comes to our knees - we just need to feel confident and trust their judgement - as long as the OS is looking out for our best long term goals.

For sure, for sure! There was a little thing I used to tell my staff when they got confused over a new surgeon's preferences.

Every surgeon's technique and style is a compilation of

All the bosses he ever worked with
All the papers he's ever read
All the conferences he's ever been to
All the discussions/debates he's ever had with his colleagues and friends be it in the seminar room or over a pint!
And all the new notions, ideas and experiences he personally has had over the span of his career.

All of which ends up making a very unique individual.
 
Hey Glousterman I was wondering how you making out sounds like you have got yourself a good surgeon there but I would not even think about the MUA at this time. I agree with Marianne I think that it really depends on the surgeon and I also believe that they would prefer for you to get there yourself. Ask me how I know LOl. I'm still hanging at a bit more than 90 degrees myself but suspect a big decision on Tuesday. Hang in there Bud you will be fine.........
 
We had a bit of a happy dance on Friday at PT - We - well I- may have broken that 90 mark finally - I will know for sure on Monday when my regular pt is back to measure - but the other pt measured me at 98. That would be an 8 degree gain this week! I hope its true - she seemed to measure different and she did mention that it may differ somewhat from the other guy - but still - even if I only made half that - 4 degrees - It would still be very exciting. Hang in there! Eventually we will all get through. At PT and even the OS keep telling me that we all heal at different rates - while I may talk (and even type) fast - I may not heal fast! It is just taking me more time - as long as we make the smallest improvement - thats a positive!
