Smith-Peterson Vitallium mould arthroplasty
A 63 year follow-up by J. F. Baker, M. H. Vioreanu, H. A. Khan, Cork University Hospital, Cork, Ireland

A 91 year old woman attended A&E complaining of right groin pain after a fall whilst getting out of her car a week before. A simple xray showed an intracapsular fracture of the right hip. However, on the left hip, a Smith-Peterson mould arthroplasty was seen.

It seems that in 1949, at the age of 29 years, she had travelled to London to undergo surgery on her painful left hip resulting from a congenital dislocation. She received the SP mould arthroplasty, the operation being performed by Mr W. A. Law.

Post-operatively she spent two weeks in traction and a further two months in hospital. She returned home four months after her operation and had a single follow-up appointment at six months. Thereafter the hip remained totally pain-free.

She noticed only a gradual development of leg-length discrepancy over the years because of which she used a heel raise. For the five years preceding the fall, she had been using a stick in her left hand when walking outdoors. This represents the only indication of functional decline during the 15 years since an earlier report of her case.

On examination of her left hip, flexion was to 90 degrees, adduction 20 degrees, abduction 30 degrees and internal and external rotation >20 degrees. The left leg was short by 6cm. She was able to demonstrate a straight leg raise against gravity. Her Harris Hip Score was 87 and Oxford Hip Score 43.
