Dr Differences of opinion for THR revision

Hi. My wife had a THR revision 16 years ago and is now experiencing increased pain, and so has very recently seen two Drs to schedule some fixing.

Dr1 took xrays, and he says we must replace the polyethylene cup as it is wearing thin, and remove the wires (I think they are wires) and trochanteric claw (I think that is what it is called). He is concerned it may fail soon and he thinks he can just replace the liner part of the socket.

2nd opinion Dr2 took xrays, and he says do not replace the cup, it is fine. The pain is coming from the clamp and wires - he would just remove those.

Dr1 does THRs as well as other surgery. Dr 2 specializes in THRs. We have xrays from each Dr, and cant see what they are seeing that makes this difference of opinion.... Currently my wife is planning to see another Dr to help figure out which way to go.

Does anyone have any suggestions or understand what might be causing this difference of opinion? Thanks very much for any help!

I think you are very sweet for asking about your wife.
As I was reading , I was thinking the same thing your wife is doing. Going for the 3rd opinion.
At this point, I would find a really well known joint replacement center or great well known doc to see what he says. I don't know what parts , besides the cup that you are speaking of , but don't worry, our wonderful nurse Josephine will surely know and answer you.

Do you have the xrays? Can you post them here?

I suggest you get another opinion and this time from a revision specialist. Either could be right. It's a difficult situation to judge and needs a very experienced eye on it.
 
Thanks for the replies! Dr2 had real film Xrays, and I can't get those on here, unless maybe he also has electronic copies. Dr1 gave us a CD with 4 different hip Xray images on it - we can only read them it seems with the program it comes with, ie, Sectra Image Viewer. Those on the CD are not as clear as the film Xrays Dr2 used, at least on our computer screen.

There are files I can see when I try to attach from the CD they are on, but I don't know if you will be able to see them... They are .DCM files and each .DCM file is in a folder with another file in there. I am attaching one of them plus the small file in the same folder (at least I think I attached both of them...). Each .DCM file is about 7.4 MB and the other file in the same folder is just a few kb.

We will check on the revision specialist suggestion, not sure how to best search for them, I guess you just search around, maybe there is a listing somewhere.

Just try ringing round different hospitals, speak to the departments of joint reconstruction or replacement and ask them if they know of any revision specialists in the area. Or you could try asking your own doctor if he can help. Perhaps some of the other members from the US will offer some suggestions.
