28 year old need hip replacement

I am a 28 year old from the caribbean. I have had surgery for dislocated hip when i was 11 years. A metal plate was put in and 3 years later removed. I now suffer with extreme pain from my lower back down to my knee. I have been turned down by insurance companies on the island and i need a hip replacement surgery badly. Can anyone tell me if there are school that need people to work on as i will resort to anything cause i cant afford the surgery. Please help me
