PKR Clunky Knee

Hi all good people on bonesmart.
Quick update. Still have not received my appointment yet for my SPECT CT scan. I am still wearing my T.E.N.S machine while at work. On my second machine now. My pain level has slowly got better but by no means free. Still taking pain medication but managing ok.
Swimming 2-3 times a week which helps a lot and still do physio at home but not as much. I am on Holiday again in September. If i haven't heard anything by then i will give them another call.

Hi good people.
Here is a quick update as to where i am with my troubled right PKR Knee. Currently i am on holiday is beautiful Cornwall Its a great part of the country i could imagine retiring here to be honest.
My knee is still causing me issues it still crunch`s a cracks a lot during flexion this is not particularly painful, walking is still my biggest challenge after maybe 5 minutes of walking it begins to stiffen and cause me discomfort then pain. I can cycle and swim with little or no discomfort it is only weight bearing that is still difficult.
I am 17 months Post op. Still waiting my appointment for a SPECT scan on my knee. I am also due to revisit my surgeon in early November i cannot see there will be any significant change up to that point.
This continues to be a difficult journey and i am not sure what will be next.
The good people on this forum continue to give me hope for positive outcome thanks again for your support.

@Ginger1210 So sorry you are still having issues with this knee! Even at 17 months that knee is still settling. Please let us know how the scan and your appointment with the surgeon goes.

Thanks Jaycey for the support. It is appreciated. It is frustrating my knee still dictates my activities but their is very little i can do at the moment. My surgeon is still concerned and has not signed me off as such. If this does happen then i will look for a second opinion.
Finding one may be difficult living in the UK. West Yorkshire to be specific. I will have to wait and see.

Hi all good people. Well after a pretty good week back at work returning from holiday. I had a stupid fall on Friday knight in my bedroom. At first i didn't think i had jarred my knee but come Sunday boy my knee let me know it is sore and swollen. I just hope i have not done something serious. after 17 months i thought it may be more resilient but a guess not. Sat now icing and elevating.
I do have a question I do get a lot of grinding and crunching in my knee particularly on the PKR side this has increased over the months. Could this be scar tissue build up or possible arthrofibrosis.

Has the icing and elevating that poor knee helped after your fall? All of us fear a fall. You can call your doctor if it doesn't get better and he can take some xrays to make sure it's OK.

Hi Sistersinhim. Thanks for the concern. Yes the icing and elevating has helped pretty much back to where i was before my stupid fall.
I think this will always be a worry. Thankfully this time i have recovered well. Thanks again.

HI all good people. I hope everyone is keeping well. It's been a while since my last post as nothing much has changed.

I had a telephone conversation with my surgery this week regarding The SPECT scan i had and the resulting report. The report shows no signs of loosening or damage to my implant area. They have booked me in for a consultation with the surgeon This coming Thursday. I am relieved that the scan shows no issues, but still frustrated and tired of feeling pain and stiffness in my knee.
Not sure how the appointment will go. I think they will try to discharge me. I hope there are other options to look at to identify my problem. I am only 49 but my knee still rules my life.
Any advice from all you good people would be most welcome.

I understand how frustrating it is to have pain or discomfort this long after surgery and yet tests show nothing wrong. I’m in the same situation, and my surgery was 5 and a half years ago.

HI Jockette. It is a tough situation i can completely sympathize with you. I can function reasonably well but only if i take pain meds and wear my trusty T.E.N.S machine while at work.
Thankfully when i go on Thursday my wife is coming with me to help ask the correct questions. We shall see. I do know that if this is it then i will have to readjust my life work balance and look at part time work.

I hope you can get some answers at your appointment this week.

Hi All good people and Happy Christmas.
I had my one to one appointment with my surgeon earlier this month. He showed me all the results from the SPECT scan and normal X-Rays. He is frustrated at my continuing issues with pain and discomfort . The scan confirmed my implant is secure and in place. It does show a possible bone impingement on the inside of the implant that could be causing some pain. But the rest looked good. He has sent all the scans to a colleague to view as well. They may opt to put a camera in my knee to have a look but the risk of infection is a concern. We shall see. He advised to stop all Physio except for cycling and swimming. The bone impingement may be the grinding i feel when flexing the knee.
I may have to look for another independent specialist in the U.K. Not sure if any of the good people at Bonesmart know of any Knee specialist in the U.K. I live in West Yorkshire but would be willing to travel to get another opinion.

I have forwarded this request to the other staff members.

Sometimes it can be so hard to find out the source of our discomfort. I have plenty of discomfort myself. I also have a partial.

@Ginger1210 What hospitals are near your location? Who was your surgeon? We need to find someone with no connection to your current surgeon.

Hi Jaycey my surgeon was Professor Nick London he practices out of Harrogate district NHS hospital and the Duchy hospital based in Harrogate.
I live in West Yorkshire. Harrogate is a good hour away so i am willing to travel. The nearest NHS Hospitals are Wakefield. Huddersfield and Leeds.

Thanks for your assistance in this.

@Ginger1210 I did a bit of research and I suggest contacting Mr Sanjeev Anand. His NHS practice is in Leeds General. But he also has private practices in Nuffield and Spire Leeds.

If you can afford to pay for a consultation at one of the private practices I would highly recommend this route. I did this for both my THRs and then got on the NHS list for the surgery. Of course the wait time for elective surgery is horrid now. But at least you can get a second opinion on this fairly quickly.

Hi Jaycey.

Thanks for the information. I may look into this. I am just waiting for the second opinion from His colleague they may decide to go in with a camera and have a look.
I have reduced my Physio to swimming and cycling which has helped with my pain management as well.

Hi I recently had a medial right PKR just over three weeks ago. Physio going well and improving a little day by day. The only issue i have is my knee now feels clunky when i walk its like its just not lined up correctly at times. This is not painful as yet it just feels strange. Is this something i should be concerned with at this early stage?
Hi Ginger,
My left TKR was 03-13-2018. I can't advise you and only give you my experience. I also have a severe condition of a clunky knee and experienced it as soon as I arrived home following surgery. I alerted my surgeon immediately and all the following visits for 12 months. My doctor didn't think it was an issue. For almost 5 years now when I walk my left knee clunks when I lift my leg off the ground and again when it touches the ground. When the room is quiet my wife can hear my knee. I had my right TKR on 01-06-21 and it doesn't clunk/click. Don't let your doctor dismiss this issue and let him know some TKR recipients have clunky knees that don't heal. Best wishes!
